### Job overview

### Ready to Roll Up Your Sleeves and Make a Real Difference?

This is not a role for someone who wants to sit on the sidelines. We're looking for a two motivated, proactive and hands-on Transformation Project Managers who thrive on turning ideas into action, tackling complex challenges and delivering visible improvements across our organisation.

As part of our Transformation Team, you'll lead a varied portfolio of projects that directly improve patient care, service efficiency, staff experience and financial sustainability. From service redesign and productivity initiatives to major transformation programmes, you'll be at the heart of helping our services work better for patients and staff alike.

You'll be someone who enjoys managing multiple priorities, building strong relationships with clinical and operational teams and maintaining momentum from concept through to delivery. You'll be equally comfortable analysing performance data, facilitating workshops, challenging existing ways of working and supporting colleagues to develop solutions for themselves rather than simply providing the answers.

These roles offer the opportunity to work closely with senior clinical leaders, operational managers and executives while remaining hands-on in delivering meaningful change across the Trust. If you're energised by variety, enjoy solving problems and want to see the tangible impact of your work, we'd love to hear from you.

This is a highly collaborative role and requires regular on-site presence for a minimum of three days per week. You'll be visible within services, building relationships, supporting teams, facilitating improvement work and helping to drive change where it matters most.

Posts available:

- 2 x full time 37.5 hours per week - 3 days on site min (5months initially with potential to extend)

As our Transformation Project Manager, you'll take ownership of a diverse portfolio of projects, ensuring ideas become reality and improvements deliver lasting benefits.

You will:

- Lead multiple transformation, service improvement and productivity projects from inception through to successful delivery.
- Work alongside clinical, operational and corporate teams to identify opportunities, solve problems and implement sustainable improvements.
- Manage competing priorities and multiple workstreams simultaneously, maintaining focus on delivery, outcomes and benefits.
- Develop project plans, business cases, governance papers and executive-level reports that support effective decision-making.
- Facilitate workshops, improvement events and stakeholder meetings which bring people together to create practical solutions.
- Use data, insight and performance information to identify opportunities for improvement and support evidence-based change.
- Provide constructive challenge, support and coaching to project teams, helping colleagues build confidence and capability to drive improvement themselves.
- Support major Trust priorities including transformation programmes, productivity improvements and cost improvement initiatives.
- Build strong and effective relationships with stakeholders at all levels, from frontline staff to senior leaders, creating engagement and commitment to change.
- Identify, manage and mitigate risks, issues and dependencies to ensure successful delivery.
- Act as a trusted partner to services, helping teams navigate change and overcome barriers while maintaining ownership of their improvement journeys.

### What Makes Someone Successful in This Role?

We're looking for someone who:

- Loves getting stuck in and making things happen.
- Thrives in a fast-paced environment with multiple projects running at once.
- Is naturally organised and able to maintain focus across competing priorities.
- Can influence and engage people at all levels of the organisation.
- Enjoys solving problems and finding practical solutions.
- Brings energy, enthusiasm and resilience when faced with challenges.
- Supports and empowers colleagues rather than doing everything for them.
- Builds credibility quickly and develops strong working relationships.
- Wants to make a genuine difference to services, staff and patients.

### Why Join Us?

This is an exciting opportunity to join a team that is driving real change across the organisation. You'll have the autonomy to lead meaningful projects, the opportunity to work with senior leaders and frontline teams alike, and the chance to see the direct impact of your work.
