Transfer of Care Team and Hub Lead - Band 7 - 37.5 hours

The post holder will be responsible for managing the team which supporting hospital discharges. The post holder will be responsible for ensuring the timely assessment and appropriate discharge of complex patients from hospital. This will be done by the completion of assessment, care planning and integrated working responsive to patient and carer’s needs. The post holder will be responsible for offering clinical leadership within the boundaries of the specific profession and to offer managerial and operational support to registered and non-registered members of the team. The Transfer for Care Team and Hub lead has a fundamental role in ensuring the caring environment is conducive to wellbeing and the effective delivery of budgetary, workforce and resource management

### Main duties of the job

To provide  effective leadership, monitoring of professional and care standards, and effective use of resources at ward/department level. To manage the TOCT and TOCH, deputize for Operational Lead of Discharge Services when required. Support and develop health and social care integration  Responsible for ensuring the provision of compassionate, safe and high-quality patient care. Lead by example and act as a positive role model to staff taking responsibility for own professional development. Promotes patient focused care. Actively contribute in the business group with  focus for supporting clinical effectiveness and innovative practice. To manage roster allocation for present and future shifts. Escalate concerns about patient safety, quality and experience. Ensure national and local policy and guidance are delivered and maintained. Foster a culture which encourages staff to challenge and report Contribute to the development of and support delivery of the Trust Organise / plan daily workload. Contribute / participate in audits,  utilisation of results in practice. Good communication with the patient and families.  Ensure effective communicate with the wards/ teams. Risk assess to ensure incidents, near misses are reported and participate in investigation of incidents complaints. Act as advocate for patient and their family, and promoting /protecting their interests, rights, equality, inclusion and privacy. Ensure all documentation is appropriate, accountable and defensible

In conjunction with the Operational Lead for Discharge Services, provide effective leadership, monitoring of professional and care standards, and effective use of resources at ward/department level. To operationally manage the Integrated Transfer Team and deputize for Operational Lead of Discharge Services when required. Support and develop health and social care integration across sharing responsibilities for leadership and management of staff in multi-professional teams. To be a visible and accessible presence in the clinical area for patients, relatives and staff. To be responsible for ensuring the provision of compassionate, safe and high-quality patient care. Lead by example and act as a positive role model to staff taking responsibility for own professional development. Provide leadership that promotes patient focused care. Actively contribute to and support agreed developments within the business group with  focus for supporting clinical effectiveness and innovative practice. To manage roster allocation for present and future shifts in order to provide safe staffing levels in line with the agreed workforce model. To lead the multidisciplinary team on a daily basis using resources effectively and efficiently. To confidently escalate in a timely manner concerns about patient safety, quality and experience, offering solutions and evidence-based recommendations.

If successful this vacancy will require a Standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
