They will be responsible for the development and delivery of discharge services. The post holder will work in line with agreed objectives reflecting the targets, quality standards, resource controls and constraints and will contribute to delivering the management strategy of the discharge service within the Trusts. The successful candidate will be a skilled communicator and be able to communicate highly complex and sensitive information at all levels across multiple Organisations. The post holder will be able foster strong relationships at senior levels with key strategic and delivery partners, across the NHS and wider health and care sector and expert a creating co-operation and collaborative in working across organisation boundaries.
