As the Transfer of Care Hub Clinician, you will provide a consistent presence within the acute settings, Poole and Bournemouth Hospital on behalf of Dorset HealthCare for the Transfer of Care hub.

You will be responsible for clinically triaging referrals sent to single point of access (SPA) within Dorset Healthcare Trust and liaising with all teams involved in planning discharges and agreeing patient pathways.  Although you will not be delivering clinical duties you will be vital in decision making to ensure safe, effective discharges across Dorset.

This is a Full-Time post of 37.5 hours. The post holder may be required to work flexibly to meet the needs of the service.

### Main duties of the job

Your role will involve liaising with patients and families on the ward when needed to expedite discharges and support the multi-disciplinary team as part of complex discharge planning

You will work closely with the pathway leads, acute hospital teams, social workers and community providers to ensure patients have a smooth transition onto the correct pathway contributing towards discharge and flow across the system.

To be the consistent presence within the acute hospital for the Transfer of care hub and be responsible for liaising with the acute hospital teams, providers and other services in planning discharges and agreeing patient pathways.

To triage referrals sent to SPA working with the central SPA team and liaise with Patients and Families on the wards where needed to expedite discharges.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.

### Job Specific Experience

- Experience at Practitioner Band 5 level
- Recent previous experience within a comparable role

### Knowledge, skills and training

- Registered practitioner to degree/diploma level supplemented by post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty
- Membership of the relevant Professional Body

**Desirable**

- Knowledge and understanding of Trust Strategy relevant to role
