Undertake patient-centred interviews which identify areas where the person wishes to see change and or recovery and make an accurate assessment of risk to self and others. Assess and support people with a common mental health problem in the self-management of their recovery. Manage and convey information which may be of a sensitive and / or distressing nature. Assess clinical risk and refer to specialist mental health services as indicated autonomously if assessed as urgent, or in collaboration with the caseload manager if the risk is assessed as low but the client's needs are beyond that of brief psychological interventions. This will involve writing and sending the referral. Assess and support people with a common mental health problem in the self-management of their recovery. Discuss patient care with external services through meetings, emails and phone calls. To maintain up-to-date knowledge of legislation, national and local policies and procedures in relation to Mental Health and Primary Care Services. Gathering patient feedback through patient experience questionnaire (PEQs). Respond to and implement supervision guidance by supervisors in clinical practice. Post holder will be responsible for maintaining continual professional development.
