### Job overview

To advise patients, staff and visitors on how to quit and / or abstain from smoking during a hospital stay and beyond.

Signpost to appropriate community support so patients can continue their quit journey following an inpatient stay.

Deliver / signpost staff training opportunities.

### Main duties of the job

- To work with the onsite Tobacco dependency Treatment Service (TDTS) and specialists to provide advice, behavioural support and treatment in relation to tobacco smoking.
- To support the day to day running of the onsite TDTS to ensure safe, well organized and evidenced based care is offered routinely to all identified smokers and provided to all that access the service. Provide support in a range of settings as required for the delivery of smoking cessation service.
- To support the delivery of the service key performance and reporting indicators for tobacco dependency.
- Organise own workload and work unsupervised in a busy team. Maintain a high standard of work and prioritise own workload according to the needs of the service, individual needs including equality, diversity and social inclusion in all aspects of work.
- Take referrals, including self-referrals, into the service and work with patients to identify the most appropriate tobacco dependency treatment pathway.
- Support individuals to make informed choices about their health and wellbeing and support them in initiating and sustaining appropriate behavioural changes to improve their health.
- Record and report activities and results.
- Develop and sustain tobacco dependency support and referrals into the service.
- Travel is required across the Trust footprint , including occasional regional travel.
- The TDTS will operate seven days each week.

### Applicant requirements

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

**Essential**

- Effective communicator

### Knowledge and skills

**Essential**

- Word processing skills including email, Microsoft office and excel.
- Ability to communicate effectively, using interpreters as required.
- Able to deliver factual information using reassurance, tact and empathy.
- Evidence of previous team working.
- An understanding of confidentiality and the ability to adhere to and implement Information Governance guidelines.
- Knowledge of tobacco treatment

**Desirable**

- Ability to use carbon monoxide monitors.

### Qualification & Training

**Essential**

- GCSE Maths and English or equivalent
- Understanding of the drivers of ill health

**Desirable**

- Level 2 smoking cessation training
