### Job overview

The Tobacco Dependency Advisor will support the delivery of a Trust-wide Tobacco Dependency Service, helping patients to quit smoking and improve their overall health.

The postholder will provide evidence-based behavioural support and treatment to patients identified as smokers, primarily within acute hospital settings, ensuring all eligible patients are offered appropriate interventions in line with national guidance.

Working as part of a multidisciplinary team, the role involves engaging patients in their quit journey, delivering personalised care plans, and supporting the implementation of effective tobacco dependency pathways across the Trust.

### Main duties of the job

Deliver smoking cessation interventions, including behavioural support and advice on pharmacotherapy (e.g. nicotine replacement therapy).

Assess patients, develop personalised quit plans, and support individuals through their quit journey in both inpatient and community settings.

Provide one-to-one and group support sessions, including follow-up care (e.g. post-discharge telephone support).

Identify and engage patients who smoke, offering evidence-based advice and promoting harm reduction approaches.

Support the implementation of the Tobacco Dependency pathway and Ottawa Model of Care within the Trust.

Work collaboratively with clinical teams and community services to ensure continuity of care and effective referral pathways.

Monitor patient progress, record outcomes, and contribute to reporting requirements and service evaluation.

Promote smoke-free policies and contribute to health promotion campaigns (e.g. Stoptober).

Maintain accurate patient records and ensure compliance with data protection and governance standards.

Contribute to service development, audits, and quality improvement initiatives.

### Specific Requirements

**Essential**

- Be able to travel independently between different sites across the Trust without the use of public transport

### Previous Experience (Nature & Level)

**Essential**

- Experience of providing healthcare, healthcare advice or health promotion advice to patients or members of the public
- Understanding of the drivers of ill health and mental health.
- Evidence of previous team working
- Confidence in working with a wide variety of partners
- Self-awareness around limitations of skills and competencies and ability to work within them

**Desirable**

- Experience of delivering smoking cessation support
- Experience of working in acute NHS secondary care sector
- Knowledge of tobacco treatment
- Experience working with people in a behaviour change context
- Experience of working in health promotion

### Qualifications (Academic, Professional & Vocational)

**Essential**

- NVQ/Apprenticeship level 3 in healthcare or equivalent experience
- NCSCT Smoking Cessation Practitioner training
- Grades A-D or 3-9 in GCSE Maths and English or equivalent

**Desirable**

- Level 2 smoking cessation training
- Specific training in health promotion/behaviour change interventions

- Good verbal and written communications skills.
- Literate in IT/Computer skills
- Work effectively and autonomously without direct supervision and/or as part of a team
- Accurate record keeping
- Ability to work flexibly to meet the needs of the service and service user.
- Confident in dealing with people, even in challenging circumstances.
- Ability to present an open and none-judgemental demeanour.
- Ability to activity listen and respond to needs of patients.
- Ability to maintain professional and confidential approach at all times.
- Awareness of the importance of taking personal responsibility and providing peer support for wellbeing.
- Ability to develop skills & Knowledge to achieve level NVQ within 2 years of joining TDA
- Ability to evidence/demonstrate key values and behaviours in line with the Trust

**Desirable**

- Experience of use of ULHT clinical systems (Careflow,ESR,ePMA)
- Knowledge of ULHT Employment Assistant Programme (EAP)
- Ability to use carbon monoxide monitors
