An exciting opportunity has arisen to apply for 30hrs/ week, for one year fixed term role as a Band 6 Occupational therapist or Physiotherapist to work at Ealing Referral hub triage adult community rehab referrals. The role also involves some out patient clinics for community rehab eg Post covid rehab. Staff will be supported with clinical supervision, Multidisciplinary team work, training and opportunity to take part in service improvement work.

We are looking for candidate to have completed core clinical rotations and able to adjust to a fast paced working environment. In this role, you will be responsible for triaging incoming referrals (from GPs, other hospitals etc..) ensuring patients are directed to the most appropriate therapy service in a timely and efficient manner. The candidate is expected to have understanding of community services with previous experience of working in the UK Healthcare. Triage work is based at the Ealing referral hub  at St Bernard's  Hospital site ( UB1 3EU) and clinics are held in one of our community sites in Southall.  Staff will need to be  flexible to support triage through Monday to Friday and work in clinics based on estates availability.

### Main duties of the job

- Work independently at the referral hub to triage all referrals for community services received at Single point of access.
- Offer advice and guidance on telephone for patients, referrers and carers as required.
- Able to prioritise and triage based on needs based assessment
- Be able to take succinct and clear documentation to support triage decision and their clinical work.
- Carry clinical case load in out patients community rehab eg assessment and management of long term conditions eg Fall service, post covid rehab.
- The post holder will work along side referral hub and other clinicians from community services to offer clinical triage and allocation to respective services.
- Support audits and training of other staff
- Maintain concise clear accurate documentation
- MDT Working
- Writing reports and letters
- undertake clinical assessment via telephone/video call as required to facilitate effective clinical decision making
- Working with DSE/ Computers for electronic triage on System One

### Detailed job description and main responsibilities

- Undertake clinical triage of therapy referrals made to the Referral Hub deciding on the correct care pathway for that patient
- Carry clinical caseload to support community rehab eg Long covid rehab, fall service etc
- To work collaboratively within a multidisciplinary team
- Work in partnership with Referral Hub staff and management to ensure that patients are allocated to the correct team in a timely way
- Liaison with acute hospitals, local authorities and other stakeholders to obtain information and resolve queries
- Participate in the ongoing monitoring and evaluation of the therapy triage, ensuring that all required data is collected, undertaking audit as required.
- To provide an evidence-based assessment, treatment and advisory service to clients with a variety of conditions.

## Person Specification

### Knowledge/ Experience

**Essential**

- Trained or worked in the NHS
- Experience of doing clinical triage
- Broad under graduate experience in Core clinical areas: Orthopaedics/ Long Covid/ Community Therapy/ Neuro/ Care of Elderly
- Management of Long term condition
- Awareness of audit and evaluation
- Understanding of Clinical governance and its application to work area

**Desirable**

- Working knowledge of community services
- Active contribution to training others

### Education and Qualifications

**Essential**

- Degree OT/ PT and current professional registration
- Continous CPD

**Desirable**

- Supervisor qualification/experience
- Relevant Post graduate training
