The trust along with system partners are designing and developing 6 Integrated Neighbourhood Teams across the city of Coventry. This involves multi-disciplinary teams working together locally to embed joined-up care, reducing hospital demand, and empowering communities for better prevention and access. Please note this role is 12 month secondment/fixed term opprtunity As the group is currently trialling a new Integrated Neighbourhood Team (INT) model following a collaborative event with internal and external stakeholders. Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP. ### Main duties of the job - To work as a Technical Instructor within the Integrated Neighbourhood Teams (INT), as a member of an interdisciplinary team. - The post holder will provide reablement to adults in Coventry under the direction of a team of qualified therapists. - This includes working with and performing tasks delegated by qualified members of the team including Physiotherapists, Occupational Therapists, and Assistant Practitioners. - To work flexibly to ensure appropriate service provision over weekends and bank holidays between 8am-4pm - To adhere to the lone worker policy. - There is an ongoing requirement for travel with this post - The post holder will be required to have a full driving licence and car and to be insured for business use. ### Detailed job description and main responsibilities - To have knowledge and understanding of a range of medical, surgical and orthopaedic conditions. - To deliver delegated therapeutic assessment or treatment according to the client’s goals and complexity, with co-workers or as a lone worker. - To regularly visit patients in their own homes and other community settings. - To assist in moving and handling and therapeutic positioning of patients to provide assessment and intervention. This may include assisting the patient getting up and down off the floor, managing stairs and working in confined spaces. - Under the direction of the qualified therapists, provide specific elements of a holistic treatment / rehabilitation programme to an assigned caseload of patients. - To assist in meeting patient’s needs during therapy visits as required.
