As part of the Multi-Disciplinary Team (MDT), you will work under the guidance of allied health professional (AHP) colleagues to deliver rehabilitation  care plans in patients’ own homes. You will help patients regain independence and maximise their rehabilitation potentials  by implementing therapy programmes, prescribing appropriate equipment, and monitoring progress to ensure care is personalised and effective. The CCP pathway supports Greenwich residents with early discharge from acute hospitals by providing timely, intensive rehabilitation in the community. The service aims to help patients remain safely at home, improve quality of life, reduce reliance on care packages and minimise the risk of hospital readmission. Due to the nature of the service, rapid interventions are often required and may need to be delivered at short notice to meet urgent rehabilitation and discharge needs.
