### Main duties of the job

The main focus of the Team Leader role at Avery Healthcare is to lend support to the home management team in delivering high-quality care that makes a difference by ensuring a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Responsibilities include leading in the person-centred care planning process, smooth transition of discharges from the home, and monitoring the well-being of each resident to ensure all needs are met and records are updated accordingly. Additionally, safe systems of work across various tasks must be practiced, assessing risks, and ensuring the safety of everyone involved. This role demands living by the values of being caring, supportive, honest, respectful, and accountable. Ideal candidates should have an NVQ 3 or equivalent in care, two years experience working in a senior position alongside the resident group, excellent verbal and written communication skills, and the ability to lead staff in the absence of the manager.

ABOUT THE ROLE

Your focus asTeamLeaderwill be tosupportthehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,supportive, andinclusiveenvironment for residents, visitors,and staff.

Other responsibilitieswillinclude:

- Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
- Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
- Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
- Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
