Job Summary / Main Purpose

Provide clinical specialist expertise in the assessment and treatment of adults and children referred to Occupational Therapy with upper limb/ hand injuries and other conditions affecting the upper limb.

As team lead, the post holder will oversee the provision of assessment and treatment of outpatient and inpatients referred to the OT Hand therapy service in Musgrave Park Hospital.

The post holder will be responsible for the day-to-day management, co-ordination and provision of the Occupational Therapy upper limb /hand injury service.

The post holder will provide leadership through supervision to staff and students and collaborate with management to ensure efficient and effective utilisation of resources.

The post holder will be required to assist, support and provide advice to other Occupational Therapy staff across the Musgrave Park Hospital site.

In response to continued service development, the post holder will be required to undertake flexible working including extended working and seven day working as determined by operational requirements.

Main Duties / Responsibilities

For each of the following, the post holder will;

### Clinical

Be responsible for and ensure delivery of a comprehensive Occupational Therapy Service to outpatients and in-patients referred from the Orthopaedic Upper limb Consultants, Paediatric Orthopaedic Consultants and Orthopaedic Integrated Clinical Assessment and Treatment Service.

Provide specialist clinical leadership to the Occupational Therapy staff working in the Upper Limb and Hand therapy team, ensuring that the highest standard of assessment and treatment is provided.

Work autonomously to undertake specialist assessment and treatment of own caseload of patients referred to Occupational Therapy for complex post-operative and conservative management, including static, dynamic and static progressive splinting and ongoing hand therapy in functional rehabilitation.

Demonstrate expert knowledge in providing therapy to patients with life changing/painful conditions e.g. Complex Regional Pain Syndrome, severe advanced arthritic conditions and Brachial Plexus Injuries

Provide baseline psychological support and appropriate signposting to patients following trauma and other complex hand conditions

Advise patients and carers about the management and use of splints, wound care management and hand therapy exercises, communicating sensitive information in a clear, concise and empathetic manner.

Attend clinics and liaise regularly with consultants and members of the multidisciplinary team to ensure common goals and outcomes are met.

Manage own complex caseload and be responsible for managing the Occupational Therapy Hand and Upper limb Service caseload, including prioritisation, allocation and monitoring of cases, ensuring high clinical standards.

Develop patient centred comprehensive treatment plans demonstrating advanced clinical reasoning, and problem solving skills, liaising regularly with consultants, patients and professional colleagues regarding hand therapy.

Provide specialist clinical leadership to the Occupational Therapy staff working in upper limb and hand injuries team; ensuring the highest standard of assessment and treatment is provided.

Liaise with the Occupational Therapy Assistant Service Manager regarding any issues relevant to the upper limb and hand injury service.

Educate patient, family/carers and ward staff regarding safe use of bespoke splints.

Communicate complex information and therapy outcomes in a clear, concise and empathetic manner, formally and informally.

Prioritise work, allocate and monitor the caseload of staff.

Provide day-to-day supervision of staff on formal and informal basis.

Be professionally accountable for the direct patient care provided by self and staff for whom responsible.

Be responsible for maintaining accurate, contemporaneous clinical records in line with the Royal College of Occupational Therapists, Health and Care Professions Council and local standards of practice.

Ensure that their own practice and that of staff under their supervision is in keeping with the Royal College of Occupational Therapist’s Professional Standards of Practice and meets the required professional, national and local standards of Occupational Therapy practice.

Support the Occupational Therapy Assistant Service Manager in the provision and development of the Occupational Therapy Service in Upper limb and hand injuries within Belfast Trust.

Work as an autonomous practitioner adhering to the Code of Ethics and Professional Conduct for Occupational Therapists, Professional Standards for Occupational Therapy Practice and Heath and Care Professions Council (HCPC) Standards

Be professionally and legally accountable for all aspects of professional and clinical work including management of patients/clients in your care.

Review, update and monitor local Occupational Therapy standards of practice in line with RCOT Professional Standards for Occupational Therapy Practice

Be responsible for maintaining accurate, contemporaneous clinical records in line with RCOT, HCPC and local standards of practice

Identify personal training needs, undertake regular continuing professional development and participate in Individual Performance Review, both as an appraisee and appraiser.

Provide leadership in the development and implementation of policies and guidelines within area of work and as delegated by senior management.

Promote and develop evidence- based practice within the service by leading in practice development activities including audit and research projects.

Communicate results and recommendations of practice audit to Occupational Therapy colleagues and other health professionals

Provide training, including internal in-service and external at undergraduate and postgraduate levels to Occupational Therapy and other health professionals in areas of clinical expertise

Demonstrate on-going professional development through attendance and participation in internal and external development opportunities in order to maintain a high level of skill to practice at an advanced level, and increase knowledge of specialised techniques and improve professional standards.

Maintain a continuing professional development portfolio as required by the Health Professions Council

Act as a Practice Educator for Occupational Therapy students

Participate in clinical research or trials, as requested by the consultants or as delegated

Act as a clinical resource, demonstrating specialist Occupational Therapy knowledge and skill in the assessment and implementation of evidence based interventions in the care of clients with upper limb/hand injuries including the integration of research evidence into practice.

Promote and maintain a high quality service, which demonstrates a sound knowledge of clinical governance and risk management.

Deputise in the absence of Assistant Occupational Therapy Manager upon request

Participate in the management/administrative processes within the Trust, including but not limited to recruitment and selection, induction programmes, capability and disciplinary proceedings, and management of attendance.

Report to the Occupational Therapy team lead on a regular basis any new developments, issues or problems within the service

Bring to the attention of Assistant Service Manager any observed shortfall or unmet need in provision to patients/ carers together with any suggested measures to improve and further develop services.

Manage and prioritise patient and non-patient related professional demands

Delegate appropriate responsibility to staff within remit demonstrating effective decision making, whilst retaining overall responsibility and accountability for outcomes.

Provide and maintain accurate and appropriate statistics and workload records including outcome measures and data on quality assurance and other agreed standards.

Raise awareness of the contribution of Occupational Therapy within area of responsibility representing the profession at management meetings as requested

Record patient and non-patient activity data in accordance with Professional and Trust standards and produce reports relevant to clinical setting.

Maintain sufficient IT skills for the demands of the job for example, keyboard skills, use of e-mail, Internet and data bases, support the use of internal IT systems such as Encompass and Microsoft Teams.
