A team support worker will provide care and support for patients and service users. You will be involved in implementing care plans and monitoring patients, reporting back to the relevant professional. You will be required to work in partnership with people with learning disabilities, their families and carers to support them to live successfully in the community. ### Main duties of the job Working part time (17 hrs per week, days negotiable) the successful candidate will already have experience of working in a public facing setting.  The role will involve working within a multi-disciplinary health and social care team to support people with learning disabilities to live successfully in the community.  You will be committed to improving the quality of life for our service users and have excellent interpersonal and communication skills.
