Part Time Band 3 Support worker– North Dorset Community Learning Disability Team

Are you an enthusiastic, forward thinking individual who can adapt to changing priorities and deliver high quality healthcare?  The Learning Disability Service in Dorset Healthcare NHS Foundation Trust is staffed by a passionate and dedicated team and is looking for a like-minded individual to join us.

A team support worker will provide care and support for patients and service users. You will be involved in implementing care plans and monitoring patients, reporting back to the relevant professional.

You will be required to work in partnership with people with learning disabilities, their families and carers to support them to live successfully in the community.

### Main duties of the job

Working part time (17 hrs per week, days negotiable) the successful candidate will already have experience of working in a public facing setting.  The role will involve working within a multi-disciplinary health and social care team to support people with learning disabilities to live successfully in the community.  You will be committed to improving the quality of life for our service users and have excellent interpersonal and communication skills.

- Carry out assigned observations and assessment of People with Learning Disabilities which may include people with complex and challenging behaviours.
- To assist Clinicians in the team in delivering direct interventions and coaching to staff teams to enable People with Learning Disabilities to meet their identified health needs
- Work alongside Clinicians as directed to support Transition of young people with a learning disability into Adult Services.
- Work alongside Clinicians to provide health facilitation services.
- To provide specialist health interventions under direction and supervision from Clinicians.

### Driver

**Essential**

- Must be a driver and have access to a vehicle

### communication

- Ability to listen/ communicate effectively at all levels

**Desirable**

- An ability to act calmly in emergencies and respond in a professional manner to stressful and challenging behaviour which can be frequent

### Computer literacy

**Essential**

- Good basic IT skills to ensure accurate completion of electronic patient records and recording activity and outcoming appointments

### Relevant experience

**Essential**

- NVQ 3 qualification or relevant experience
- Ability to work with minimal supervision in a range of settings
- Must have knowledge of basic administration
- Experience of working with people with a learning disability

### Organisational skills

**Essential**

- The ability to hold a case load of clients with variable issues and work closely alongside CLDT.
