## Job Content

Riverbank is looking to recruit a Support Worker to join the team. This is a full time, 37.5 hours role which consists of 3 shifts a week. Our shifts are 12.5 hours long and are between 08:00-20:30 and 20:00-08:30.Riverbank is located in Hull, in the East Riding of Yorkshire. The service offers residential support for 24 males and females with mental health needs. We are located off a main road leading to the city centre and within walking distance of a beautiful park.Our team are dedicated to supporting the people we care for to be able to live independently in the community, increasing their quality of life. Riverbank offers a step down service from hospital or locked rehabilitation units and also supports individuals who may require a step up from the community, in cases where more support may be needed to prevent relapse and potential hospital admission.We can also support individuals with:Complex needsA forensic backgroundA dual diagnosis, with a secondary diagnosis of a learning disability or those on the autism spectrum

- Provide personal care and support with daily living tasks.
- Encourage and assist participation in activities and community life.
- Support service users in achieving their personal outcomes in the way they choose.
- Maintain accurate records, care plans, charts, and daily notes.
- Build positive relationships with service users, families, colleagues, and stakeholders.
- Uphold safety, security, and confidentiality in line with policies and regulations.
- Act as a key worker for individuals, helping them to set goals and track progress.
- Promote choice, independence, and involvement at all times.
- Contribute to continuous improvement by sharing ideas and suggestions.
