### Job overview

We are seeking an experienced and motivated Health & Justice Navigator/Support Worker to join Sussex Liaison & Diversion Service (SLDS). Working within Police Custody settings across Sussex, you will assess adults, children, and young people who may be experiencing mental health difficulties, learning disabilities, substance misuse, or other social vulnerabilities. Your interventions will help ensure individuals are identified early, connected with the right services, and supported to achieve better outcomes. This is a varied and rewarding role where no two days are the same. You'll work closely with police, courts, healthcare professionals, and community partners, using your clinical expertise to influence decision-making and ensure people receive the right care at the right time. We're looking for an compassionate individual with excellent assessment and risk identification skills, a collaborative approach, and a passion for improving lives.

### Main duties of the job

- Deliver high-quality assessments for adults, children and young people in Police Custody settings across Sussex.
- Identify mental health, learning disability, substance misuse, physical health and social vulnerabilities at the earliest opportunity.
- Develop and implement person-centred care and support plans to meet complex needs.
- Make timely referrals to appropriate health, social care and community services.
- Work closely with police, courts and criminal justice partners to support informed decision-making.
- Provide accurate information regarding an individual's health needs and vulnerabilities to relevant agencies.
- Use a trauma-informed approach to minimise distress and avoid individuals having to repeatedly share their experiences.
- Promote positive outcomes by helping vulnerable individuals access the right support at the right time.

## Person Specification

### Experience

**Essential**

- Experience of working with a range of vulnerabilities which may include mental health, physical health, learning disabilities, abuse, addiction and social issues.
- Experience of working with a range of statutory, non-statutory agencies.

**Desirable**

- Lived Experience of vulnerability, criminal justice systems and / or leaving care.
- Experience and/or sound knowledge of the Criminal Justice System.
- Experience of working in a community based setting.
- Experience of implementing personalised care plans or rehabilitation plans.

### Qualifications & Training

**Essential**

- NVQ Level 3 Health & Social Care (or willingness to work towards) or equivalent experience.
- The post holder will require additional vetting by partner agencies; this includes Police Vetting and / or Prison Clearance.

**Desirable**

- Training in Supporting / Mentoring / coaching
