Working as part of a supportive multidisciplinary team, you will provide ongoing support, therapeutic interventions, practical assistance, and problem-solving to service users under the supervision of a senior team member. The Community Forensic Service delivers a 7-day-a-week service, supporting individuals in their recovery journey while promoting wellbeing, independence, and effective risk management within the community. No two days are the same, making this an ideal opportunity for someone who enjoys working autonomously while building meaningful relationships with service users. This is a fantastic opportunity for an organised, motivated, and compassionate individual who thrives in a dynamic environment and is passionate about supporting people on their recovery journey. ### Main duties of the job The post-holder will work with service users and the multi disciplinary team in an enthusiastic and conscientious way to ensure the delivery of high quality recovery focused care and support. The role will require a real desire and ability to engage with service users in a confident, compassionate and non-judgemental manner, with a primary focus of enabling recovery, well-being and independence. The service works with adults with diagnoses of mental health conditions and/or learning disability who present with a risk of significant harm to others, to achieve optimal management of risk and minimize re-offending. The service adopts a compassionate and trauma informed approach to promote relapse prevention and recovery, so enabling individuals to realise their full potential and support them to continue to live and build meaningful lives in the community. You will be joining an experienced team of professionals (psychiatrists, psychologists, nurses, occupational therapists, support workers, administrators) who all share a passion and drive for delivering high quality care and treatment to service users with Forensic histories. The service runs 7 days a week (8am till 8pm), excluding bank holidays, and will include shift work on a rotational basis. ### Detailed job description and main responsibilities Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please refer to the attached job description and person specification for full details of responsibilities To support the users of the Forensic Psychiatry service with a range of practical, health and social issues. Implement and maintain psychological and occupational therapy interventions under the guidance of qualified clinicians, monitoring progress and providing feedback to support positive outcomes for service users. Work with service users in a variety of settings to facilitate access and engagement in a range of community based activities. Develop positive, trusting relationships with service users while maintaining clear, appropriate, and professional boundaries. Help users gain access to resources including benefits and welfare rights. Help to identify early signs of relapse by monitoring users progress, level of functioning and mental state and alert the appropriate staff involved in the individuals care. To attend meetings internal and external to the Trust when deemed appropriate by the senior clinician involved or team manager. Maintain adequate records as required by existing procedures, entering appropriate details on the service users case notes as necessary. Attend and participate in team/care plan review meetings as appropriate. Develop good working relationships with other organisations that are able to help individuals achieve their goals, including colleges, schools, other training providers and voluntary agencies, the inpatient units and community services.
