### Plus Distant Islands Allowance of £1,461 per annum

We are seeking an enthusiastic and committed physiotherapist to join our Short-Term Assessment and Reablement Team.

The Western Isles Health and Social Care Partnership is delighted to extend their investment in intermediate care services with the expansion of the Short-Term Assessment and Reablement Team (START). The START service combines the skills and expertise of social care and Allied Health Professional staff to support individuals to restore and maintain their independence. The Service is valued by families and professionals working across health and social care and we welcome applications from individuals wishing to join our Team.

This exciting role is part of a small multi-disciplinary team comprised of Reablement Workers, Care and Support Supervisors, Occupational Therapists and is also part of the Physiotherapy Department, Western Isles Hospital for peer support and ongoing CPD.

The post holder will work with people in their own home or in a homely setting to support them through a short term reablement programme and with the Community Physiotherapy team. This role plays a fundamental part in supporting people to regain mobility, strength and independence, and in the area of Falls Prevention. A component of the role is support for urgent care, Hospital at Home and avoidance of hospital admission, liaising closely with the Frailty Service.

The role will include shift patterns across 7 days. A clean UK driving licence and access to a vehicle is essential as well as a willingness to travel.
