This post will have access to senior support for advice for more complex patients. - Promote the health of patients and assist in the prevention of secondary complications of patients with lymphoedema and tissue viability problems, through enabling and encouraging independence, provision of support, facilitation of advice and information and education and training. - Liaise with and advise Community Nursing staff/Care Home staff and other healthcare professionals to provide specialist assessment and personalised care planning and intervention to all patients referred to the service. ### Detailed job description and main responsibilities Please view the attached job description and person specification to view the full details about the role.
