### Job overview

- Participate in the triage of TVOS/Lymphoedema/Hub Dressing referrals and process within set Key Performance indicators.
- Work as part of a team delivering patient centred, evidence based holistic care within the specialist area of tissue viability, wound care and Lymphoedema.
- Work with primary, secondary care and nursing home staff and the multi-disciplinary team offering advice and educational input.
- Provide care provided in a variety of settings e.g. clinics, Wards, Hospice, Care Homes, Support Living and patients own homes.
- Engage with partner organisation regarding promoting the service and develop effective pathways for patients.

### Main duties of the job

- To be responsible for the assessment, planning, implementation and evaluation of individualised evidence based care with minimal ongoing supervision of senior qualified clinician.
- Demonstrate a positive and realistic attitude to the patient and their carers with cancer related and non cancer related lymphoedema to ensure that the physiological, psychosocial, spiritual and cultural needs of the patient and family are met.
- Assess the informational needs of the patient, use appropriate materials to ensure optimum understanding of the causes of the condition, its management and anticipated outcomes of treatment.
- Assess, manage, re-assess and negotiate with patients with tissue viability and lymphoedema needs, using reflective practice and clinical reasoning to identify their physical, psychological and psychosocial needs and identify treatment goals and plans. This post will have access to senior support for advice for more complex patients.
- Promote the health of patients and assist in the prevention of secondary complications of patients with lymphoedema and tissue viability problems, through enabling and encouraging independence, provision of support, facilitation of advice and information and education and training.
- Liaise with and advise Community Nursing staff/Care Home staff and other healthcare professionals to provide specialist assessment and personalised care planning and intervention to all patients referred to the service.

## Person Specification

### Knowledge

**Essential**

- Up to date clinical knowledge of current advances in wound care, pressure ulcers, lower limb wounds management and lymphoedema
- Effective Management skills
- Excellent interpersonal and written communication skills with attention to detail
- An understanding of quality, clinical risk, clinical governance, safeguarding, national benchmarks and other professional issues
- The ability to manage change effectively.
- Effective skills in presenting information
- Commitment to high quality care
- Commitment to integrated working.
- Provide evidence of ongoing personal and professional development
- Computer Literacy
- Flexible approach to work Able to work under pressure to meet deadlines

### Experience

**Essential**

- Demonstrable post qualification experience within the relevant subject
- Experience as a senior clinical nurse
- Experience of managing staff (PDR/Supervision)
- Experience of implementing change in a clinical setting
- Experience in running clinics within the community

**Desirable**

- Experience of MDT working

### Qualifications

**Essential**

- First level Registered Nurse Nursing & Midwifery Council Registration
- Recognised professional qualification in a relevant subject
- Evidence of relevant continued professional development in wound management and/or lower leg wound/lymphoedema modules

**Desirable**

- Management qualification at Certificate, Diploma, Masters or equivalent level
- Mentorship/ preceptorship or equivalent teaching qualification

### Skills/ Abilities

**Essential**

- Working towards being Nurse Prescriber
- Extended Knowledge of Tissue Viability and Lymphoedema
- Insite into research within both specialities

### Additional Requirements

**Essential**

- Access to transport and the ability to travel within the required geographical area
