As a Band 5 Staff Nurse, you'll be part of a ward that takes pride in being dementia friendly while delivering comprehensive care to our elderly patients. You'll work within a supportive multidisciplinary team that champions person-centred care and dignity for all patients.

### Main duties of the job

- Deliver specialised, compassionate care to elderly patients, with   particular emphasis on supporting those living with frailty and     dementia
- Champion our dementia-friendly initiatives and ensure best              practice  in person-centred care
- Work collaboratively within our multidisciplinary team to create and maintain a therapeutic environment
- Assess, plan, implement and evaluate individual patient care needs, considering both physical and cognitive aspects
- Support families and carers, providing education and guidance throughout their loved one's hospital journey
- Participate in quality improvement initiatives to enhance our services
- Monitor patients' conditions closely and report changes to the multidisciplinary team
- Administer medications and treatments as prescribed, maintaining accurate documentation
- Participate actively in discharge planning to ensure safe transitions of care

- Engage patients in activities within our newly refurbished day room
- Support the implementation of our Happiness Programme initiatives
- Communicate effectively with patients, families, and carers
- Advocate for patients' needs and preferences within the                                        multidisciplinary team
- Participate in family meetings as required

## Person Specification

### Qualifications, skills and Experience

**Essential**

- Registered Nurse (RN1) with active NMC registration at time of starting appointment
- Clinical expertise relevant to the area
- Able to manage patients emotional needs and demonstrate  empathy and caring
- Ability to work collaboratively with other staff members
