Entry Criteria for a Specialty Doctor: A doctor appointed to this grade:shall have full registration and a Licence to Practice with the General Medical Council (GMC);

And

- shall have completed at least four years’ full-time postgraduate training (or its equivalent gained on a less than full-time or flexible basis) at least two of which will be in a specialty training programme or specialty registrar locum appointment for training (LAT); Or
- shall have equivalent training, experience or competencies, either in the UK or overseas.

Applications are welcome for the above Permanent/, full time/ (40 hours per week) post based in Midlothian Community Hospital

Applications are invited from Specialty Doctors, General Practitioners, and other suitably experienced clinicians to join the Midlothian Hospital at Home Service, based at Midlothian Community Hospital. Applicants with experience in acute medicine, frailty, general practice, community-based acute care or related specialties are welcomed. The service provides acute admission avoidance and supports early discharge through the delivery of high-quality acute care closer to home. There is no on call or out of hours working associated with this post other than working with colleagues to ensure public holidays are covered. The post offers flexible and compressed working, dedicated Supporting Professional Activity (SPA) time and regular multidisciplinary team working within an innovative Hospital at Home service.

### Teaching

- Evidence of commitment to: formal and informal teaching and training of trainee doctors, medical students and other clinical staff

- Evidence of training in clinical and / or educational supervision

### Experience

**Essential**

- 4 years' experience in General Practice, General Medicine, Acute Medicine, Geriatric Medicine, Internal Medicine or other relevant medical specialities. Applications are also welcomed from clinicians who can otherwise demonstrate relevant experience in the management of acutely unwell adults

**Desirable**

- Recent experience managing patients in any acute medical setting
- Experience in Hospital at Home, frailty services, acute admission avoidance, community urgent care, interface medicine or equivalent services
- Experience working across primary care, community services and secondary care interfaces

### Qualifications and Training

**Essential**

- GMC registered medical practitioner
- Licence to practice
- MRCP or MRCGP or equivalent or in process of completing

**Desirable**

- The post holder should ideally have additional training, qualifications or demonstrable experience relevant to acute medicine, older people's medicine, frailty, general practice, hospital at home, community urgent care or unscheduled care. This would support the safe assessment and management of patients who would otherwise require acute hospital admission.
- Experience using electronic patient records, digital clinical systems and technology supporting community-based acute care and virtual ward models.
- Experience in medicines optimisation, polypharmacy review and deprescribing for frail or complex patients.

### Audit and Quality Improvement

- Evidence of commitment to: clinical audit or quality improvement work

**Desirable**

- Audit or quality improvement presentations at local or national level
