We are seeking a dynamic, enthusiastic and compassionate Specialty Doctor to join our Adult Home Treatment Team in CNWL Harrow.

This is a well-established, high-performing multidisciplinary service providing intensive assessment, treatment and support to adults experiencing an acute mental health crisis

### Main duties of the job

The successful candidate will be highly motivated, energetic and passionate about delivering high-quality care to adults in crisis. They will be committed to providing assessment and intervention in a sensitive, accessible and culturally appropriate way. Section 12 approval is required for this post.

In CNWL Harrow, we are committed to whole-system working and close collaboration across services. The Home Treatment Team has strong links with Harrow Mental Health Services, including CNWL Community Mental Health Services, inpatient wards and Adult Liaison Services at Northwick Park Hospital.

### Detailed job description and main responsibilities

Clinical:

- To holistically assess patients formulating individualised Care Plans with service users and their carers that aims to address their psychological, physical, spiritual & social needs, incorporating treatment and signposting / referring to other services where necessary.
- Provide medical consultation, advice and expert knowledge to all team members.
- Involvement within normal working hours in urgent assessments (which might involve Mental Health Act assessments)
- Provide support, advice, liaison and information to all who might be concerned in the care of the patient. This includes education and advice to family members/carers of patients as well as professional carers.
- To formulate risk assessments and ensuring all clients have appropriate care plans in place which addresses the risks.
- To maintain up-to-date clinical records electronically in line with agreed local policies and procedures, guidelines from professional bodies and CNWL policy.
- To liaise with carers, general practitioners, acute hospital services, social services, and third sector organisations.
- Regular medication review and inform GP’s of any changes
- To ensure all key performance indicators are maintained in line with trust policies.
- To have a good knowledge of safeguarding of vulnerable adults and report to the necessary agencies.

Management and Leadership:

- To contribute to the development of a culture of risk assessment and management to be delivered effectively in own area of responsibility and practice
- To represent the team/sector on both internal and external clinical and strategic planning forums and to act in a professional manner at all times.

The post-holder will contribute to the supervision of the trainees and is expected to contribute and support in other clinical duties and writing discharge summaries. To work as part of the multi-disciplinary team and to provide expertise within the department, upholding professional standards at all times.

You must have appropriate UK professional registration.

### Clinical Experience

- - Extensive experience in the management of a broad range of relevant mental disorders in adults

**Desirable**
