We are pleased to announce a unique and highly rewarding opportunity for a Specialty doctor to join the Hospital at Home (HAH) and Same Day Emergency Care (SDEC) services at Darent Valley Hospital. This newly created post is part of a pioneering cross-sector expansion initiative designed to enhance our established care pathways. By joining our team, you will contribute to shaping the future of innovative healthcare delivery.

The HAH, SDEC, and Frailty Assessment Unit are strategically integrated within the SDEC Village, facilitating collaboration across multiple specialties. In this role, you will work closely with our emergency department and the medical and surgical directorates to optimize both step-up and step-down care for our patients. As a member of the HAH service, which reports directly to the  executive team, you will have the opportunity to impact patient outcomes while helping to lead service enhancements.

### Main duties of the job

You will join our existing multidisciplinary team as described above. The primary goal of this post is to support and enhance the Hospital at Home and SDEC services.

You will work collaboratively and closely with the clinical lead and lead nurses in Hospital at Home, with opportunities to contribute to the development of new patient pathways, and potential to explore service developments. You will be expected to triage referrals, review patients and management plans, working seamlessly between SDEC and HAH. We expect the post-holder to be involved in the development of medical students, trainees and allied  health professionals.

### Detailed job description and main responsibilities

Engage in daily, consultant-led ward rounds to maintain continuity and quality of patient care.
- Triage and accept referrals from Same Day Emergency Care (SDEC), Emergency  Department (ED), inpatient wards, and General Practitioners, streamlining patient transitions.
- Conduct regular reviews of treatments, investigations, and care plans, ensuring comprehensive patient management.
- Contribute to the development of specialty-specific care pathways, enhancing service delivery.
- Attend and present at governance meetings to support continuous quality improvement.
- Perform clinical procedures, including bedside ultrasound, ascitic drains, and lumbar punctures, to expand practical skills.

## Person Specification

### Experience

**Essential**

- At least 12 months experience in acute general medicine
- Middle grade experience working in acute medicine
- Evidence of participation in audit and clinical governance

**Desirable**

- Experience in working in community services such as primary care

### Qualifications

**Essential**

- MBBS or equivalent
- MRCP Full
- Full Registration with the GMC
- At least 6 months experience in a medical speciality at a higher level (eg SpR)

**Desirable**

- At least 12 months experience in acute general medicine
- Middle grade experience working in acute medicine

### Knowledge & Skills

**Essential**

- Experience in relevant procedures, including: ascitic drains, chest drains and Lumbar Puncture
- Able to prioritise clinical need
- Able to work independently but aware of own limitations and the ability to ask for help
- Knowledge of the Hospital at Home Operational Framework

**Desirable**

- Experience in ultrasound
- Specialist interest relevant to acute medicine or front door medicine
- Experience and interest in Quality improvement and management
