This is a 10 PA fixed-term post for 24 months, supporting adults aged 1865 within a defined Exeter locality. This is a full-time, community-based role, although Less Than Full Time (LTFT) applications will be considered. You will be based within a well-established multidisciplinary team, with dedicated office space, IT and secretarial support available. The post holder is expected to be actively involved in formulating a care plan and treatment plan based on evidence-based treatment guidelines. The post holder is expected to work in close liaison and collaboratively with other services/agencies. The post holder would be a key member of the multi-disciplinary team. The team provides initial assessments of patients who are referred to the Service and triaged by the Referrals management service.
