### Job summary

This is a highly rewarding role that offers the opportunity to make a meaningful difference to peoples lives. The Specialist Stop Smoking Practitioner will support the delivery of evidence-based stop smoking services aimed at reducing health inequalities across the borough of Sefton. The role will include delivering stop smoking and tobacco dependence interventions throughout multiple venues across Sefton including Community centres, GP practices and workplaces. The postholder must be able to work flexibly, independently, and collaboratively as part of a team and be willing to work one evening per week to meet the needs of the service.

Part of this role will also involve occasional work with hospital settings. This will include reviewing and updating referral pathways to make sure patients are referred to the service promptly when they are discharged from hospital.

Your key responsibilities will include working with colleagues across other organisations and services to promote the service and ensure effective referral pathways, while embedding smoking cessation support as part of routine client and patient care. You will motivate and empower clients to make positive changes to address their addiction, clearly explaining the principles of nicotine dependence and the Stop Smoking Service to enable informed choices. The role involves delivering evidence-based stop smoking interventions in line with the NCSCT Standard Treatment Programme, including one-to-one, drop-in, group, telephone, and online support. You will analyse health, equality, and service data to identify local need and inform service delivery, and provide high-quality, efficient, and cost-effective support across a range of community settings. You will also lead peer support groups for successful quitters to help prevent relapse, deliver education on smoking cessation and behavioural change, provide ongoing motivational support, and work with individuals to set realistic and sustainable goals. In addition, you will ensure all client and service records are accurately maintained in line with ABL Health documentation policies.

### Experience

**Essential**

- Experience of working in community settings

### Qualifications

**Essential**

- Higher level education or health/social care/public health/ professional qualification, or experience working within smoking cessation other addiction or behaviour change.

**Desirable**

- NCSCT Practitioner
- Relevant degree

### Job Specific Requirements

**Essential**

- Driver with a full clean driving licence and access to vehicle for work purposes. Work flexibly and adapt to suit service need.
