### Job overview

The Specialist Static Integrated Discharge Team Physiotherapist (Band 6) provides specialist physiotherapy assessment, treatment and rehabilitation for patients within acute hospital settings, supporting safe and timely discharge and admission avoidance. Working as part of an integrated multidisciplinary team, the post holder manages a complex caseload, undertakes comprehensive patient assessments, develops individualised treatment and discharge plans, and promotes patient independence through evidence-based rehabilitation. The role includes supervising and supporting junior staff, therapy assistants and students, contributing to service improvement and clinical audit, and deputising for the Team Lead Physiotherapist when required. The post holder works autonomously, collaborates closely with health and social care partners, and plays a key role in delivering seamless, patient-centred care across acute and community services

### Main duties of the job

To provide specialist physiotherapy to patients within an acute setting as part of an inter-professional team. Participating as a member of the team; managing a clinical caseload within the protocols, objectives and procedures of the team.

### To support service improvement and delivery within the team. Participate in health promotion and education of staff, clients and families.

The post holder will undertake regular continual professional development activities.

## Person Specification

### Skills

**Essential**

- Evidence of CPD in previous year.
- Awareness of clinical governance and understanding of the NHS and challenges facing service delivery
- Advanced clinical assessment skills with the ability to verbalise the clinical reasoning process in the management of patients within admission avoidance/discharge planning.

**Desirable**

- Knowledge of patient centred practice.
- Specialist admission avoidance/discharge planning knowledge and interest.

### Experience

**Essential**

- Minimum 2 years B6 rotational experience
- Experience of training/supervision.
- Experience of multi-professional working.

**Desirable**

- Experience within an acute discharge planning/admission avoidance team.

### Qualifications

**Essential**

- Degree or diploma in Physiotherapy
- Current registration with HCPC
- Post-graduate qualification in a relevant field or proven equivalent experience / training

**Desirable**

- Membership of Special Interest Group
- Practice Placement Education Certificate.
