Neuro Early Supported Discharge: Neuro ESD is a multidisciplinary team that provides time-limited, intensive, specialist neuro, home-based rehab with patients in Southampton city and surrounding areas. The service supports acute hospital discharges and admission avoidance for patients following an acute stroke or other neurological event. Community Speech and Language Therapy Service: The SLT team support people in their own homes, care homes and outpatient settings across Soton and Southwest Hampshire. Patients include those recovering from strokes, people living with Parkinson's disease or dementia, and frail or older individuals who need support with communication or swallowing. Our teams are part of the HIOWH Neuro Rehab Services, you will work alongside Clinical & Team Leads and you will be supported by the Clinical Matron and Adult SLT Professional Lead. You will receive profession specific and CPD support. The post holder will need to have post graduate experience which must include some specific stroke/neuro clinical experience and evidence of working within an MDT. They must have an enthusiasm for providing evidence-based practice, particularly in the field of aphasia therapy.
