In addition you will be providing and maintaining a high standard of chronic disease management for patients with chronic diseases, which includes the diagnosis, monitoring and development of individual management plans, and agreeing these as appropriate with the patient and other health professionals. ### About us Beacon Medical Group Practice is a single practice, Primary Care Network (PCN), providing care to 43,000 patients across Plympton and the South Hams.
