The post holder will provide first-class and evidence-based patient care and promote health through patient empowerment, health education and self-management in the hospital, patients’ homes and at Camden & Islington Sickle Cell & Thalassaemia Centre. ### Main duties of the job - Provide support as a Red Cell Community key worker to a caseload of patients through regular home visits and telephone contact to improve quality of care and to support admission avoidance. - Provide specialised physiotherapy to patients who are case managed by other members of the team. - Work in close collaboration with the Whittington Health Haematology consultants, the Community Team, GPs and community services to provide a comprehensive and seamless programme of care for suitable patients. - Attend regular multidisciplinary team meetings including the hospital-based team meeting to review patient management, new referrals, and discharges; and, team meetings to share learning, reflective practice, and education. - Work as a practitioner assessing patients in their own homes, health centres and other community settings.
