The post holder will work as an integral member of the Pharmacy Virtual Ward Team within South Tees Hospitals NHS Foundation Trust, promoting the safe, rational, effective, and cost‑effective use of medicines for patients managed within the Virtual Ward. The base working site is negotiable with the successful candidate.

The role involves undertaking medicines reconciliation and making safe, appropriate clinical interventions, providing counselling and clear information to patients about their medication, and delivering pharmaceutical advice to members of the Virtual Ward multidisciplinary team with support from clinical pharmacists. The post holder will analyse and interpret pharmaceutical information, actively contribute to MDT discussions and virtual ward rounds, and provide training to MDT colleagues on the safe use of medicines and medicines optimisation to support high‑quality patient care.

## Person Specification

### Expereince

**Essential**

- Significant previous experience working in pharmacy.
- Experience working in hospital pharmacy.
- Experience of working with other healthcare professionals.

**Desirable**

- Experience and understanding of evaluating and measuring the performance of health services.

### Qualifications

**Essential**

- BTEC Level 4 Diploma in Clinical Pharmacy Services and Therapeutics, or willingness to work towards
- BTec/NVQ level 3 in pharmaceutical sciences or equivalent.
- Any national or regional accreditation of medicines management modules.
- Any national or regional accreditation of accuracy checking technician training.
- Registration with General Pharmaceutical council as a Pharmacy Technician.

- Educational supervisor qualification

- Able to deliver training to other healthcare professionals.
- Skills for communication on complex matters and difficult situations, requiring persuasion and influence.
