The base working site is negotiable with the successful candidate. The role involves undertaking medicines reconciliation and making safe, appropriate clinical interventions, providing counselling and clear information to patients about their medication, and delivering pharmaceutical advice to members of the Virtual Ward multidisciplinary team with support from clinical pharmacists. The post holder will analyse and interpret pharmaceutical information, actively contribute to MDT discussions and virtual ward rounds, and provide training to MDT colleagues on the safe use of medicines and medicines optimisation to support high‑quality patient care. ### Main duties of the job Develop, implement, and evaluate a patient centred pharmacy service to the virtual ward by supporting the supply of medicines and providing pharmaceutical care to patients in their own homes, both remotely using technology and by visiting patients in their own home.
