A fantastic opportunity has arisen for a pharmacist to join the medicine team at the Chelsea site of Chelsea and Westminster NHS Foundation Trust. We provide clinical pharmacy services to the Acute Assessment Unit, Ambulatory Care and Emergency departments, as well as a range of specialties including Endocrinology, Respiratory, Gastroenterology, Care of the elderly. The post-holder will rotate between different specialities, taking responsibility for the provision of clinical services to a specialist team.

This role provides an excellent opportunity to develop clinical, managerial and leadership skills. Our department has a strong track record in successfully using a quality improvement approach to medicines optimisation with a significant portfolio of publications and awards. We provide a supportive learning environment and actively encourage participation in research and improvement projects.

### Main duties of the job

The key responsibilities of the role include:

- To work closely with the multi-disciplinary team to provide a specialist clinical pharmacy service to patients within the Medicine Directorate.
- To provide clinical cover to AAU when necessary including participation in ward rounds.
- To support the development of junior pharmacy staff through teaching and supervision including pre-registration pharmacists and junior diploma students.
- To actively participate in improvement initiatives aimed at optimising medicines use and developing clinical services.
- To assist with directorate work including finance reports, DATIX reviews and processing homecare prescriptions.

- Provide a comprehensive clinical pharmacy service within the Emergency and Integrated Care (EIC) Directorate, ensuring the safe, effective and cost-efficient use of medicines through prescription reviews, medicines optimisation, patient counselling, discharge planning and participation in multidisciplinary ward rounds.
- Support and develop medicines management initiatives by applying specialist pharmaceutical knowledge, clinical reasoning and governance principles to improve patient outcomes and minimise medication-related risks.
- Communicate effectively with patients and wider healthcare professionals, making appropriate referrals and providing expert medicines advice when required.
- Contribute to leadership, service management and quality improvement agenda by supervising and mentoring junior staff, promoting evidence-based practice, participating in audit and research activities, supporting clinical pathway development and monitoring service performance.
- Maintain professional standards through ongoing education and training, adherence to legal and ethical frameworks, commitment to equality, diversity and inclusion, and active participation in training and continuous service improvement initiatives.

## Person Specification

### Experience

**Essential**

- Experience in the medicine speciality and in acute medicine
- Education and Training of others
- Experience as a clinical pharmacist practitioner, demonstrating appropriate level of competency as outlined in the GLCF
- Awareness of and Implementation of evidence-based medicine into practice

**Desirable**

- Undertaken clinical audits of quality improvement projects to improve patient care

### Personal Qualities

### Education and Qualification

- Vocational Master’s Degree in Pharmacy (+ completion of pre-registration training)
- Registered as a practicing pharmacist with the General Pharmaceutical Council
- Certificate in Pharmacy Practice or JBP Postgraduate Certificate in General Pharmacy Practice or equivalent training / experience

**Desirable**

- Diploma in Pharmacy Practice or JBP Postgraduate Certificate in General Pharmacy Practice or equivalent training / experience
- Independent Prescribing
