Join our dynamic and supportive Community Rehabilitation  and Intermediate Care Team as a Specialist Occupational Therapist. We are looking for a motivated and flexible individual to deliver high-quality, person-centered care to adults within their own homes and within Intermediate Care inpatient settings. This is an exciting opportunity to work within a well-established multidisciplinary team, providing rehabilitation across a wide range of conditions including frailty, long-term conditions, and chronic pain. You will also play an active role in the ongoing development of integrated neighbourhood health MDTs, working collaboratively across organisational boundaries to support coordinated, holistic care. There will be opportunities to rotate periodically between IMC ward based work and community rehabilitation.

You will manage a varied and rewarding caseload, delivering holistic occupational therapy interventions in community settings. The role focuses on promoting independence, enabling meaningful activity, and supporting individuals to achieve their personal goals.

### Main duties of the job

The department and team offers a wide range of experience. You will be encouraged and supported to develop your clinical, teaching, and leadership skills as well as contribute towards service development and service evaluation. The department has a strong commitment to CPD, a comprehensive IST programme and dedicated time for your ongoing professional development.

You will:

- Assess, plan and deliver rehabilitation programmes within patients’ homes and community environments
- Work with individuals with a diverse range of physical and functional needs
- Manage a mixed caseload including frailty, long-term conditions, chronic pain, and complex presentations
- Work as part of evolving integrated neighbourhood MDTs, contributing to coordinated, person-centred care planning
- Collaborate closely with Physiotherapists and wider MDT colleagues
- Contribute to service development and quality improvement initiatives
- You will  be professionally and legally accountable for all aspects of own work, including the management of patients in your care.
- You are expected undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies; use clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition. To plan, prioritise and deliver clinical case work.

We're looking for someone with experience in community and/or intermediate care, excellent communication skills, and a passion for driving positive change. A British driving license and access to a car are essential.

### Skills

**Essential**

- Good communication and organisational skills
- Competent in core areas of Occupational Therapy practise
- Competent IT skills

### Knowledge

**Essential**

- Understanding and experience in working in as an Occupational Therapist in a Community setting with knowledge of relevant NHS documents such as the NHS Long-term Plan.

**Desirable**

- Attendance on external post-graduate courses

### Experience

**Essential**

- Substantial post-graduate experience, including recent experience in the NHS.
- Previous experience working with and assessing adults with complex needs in a community setting
- Some experience of student supervision

### Qualifications

**Essential**

- Degree in Occupational Therapy Studies
- HCPC Registered
- Evidence of post graduate learning

**Desirable**

- RCOT
