The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing care or prevent admission with interventions that can be managed at home To provide a high standard of Occupational Therapy intervention within the Intermediate Care Service working across the Cwm Taf Morgannwg University Health Board (CTMUHB) localities, health board sites, various community settings and patient homes. To play an active role in the delivery of training programmes. With appropriate training, the post holder will be able to lead on training sessions with delegated responsibility from the Senior Occupational Therapist/AHP Lead. This posts is fixed term/secondment for 12 months to cover maternity leave. To take delegated responsibility from the Senior Occupational Therapist/AHP Lead within the Intermediate Care Service managing patients at risk of unnecessary hospital discharge delay or admission and be responsible for providing Occupational Therapy assessment and treatment plans for patients within the service. Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn’t mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates.
