The team works closely with acute and community services, providing a comprehensive assessment of patients to avoid admission. We are an integrated team supported by a social care practitioner, specialist nurses and consultant geriatricians. The service operates over seven days, between 8am and 6pm and the successful candidate will be rostered regular days of work (Monday, Tuesday and Wednesday preferred) within this. The successful applicant will be able to demonstrate a flexible approach to the assessment of older adults in crisis, acknowledging the broad aspect of medical and social factors influencing frailty.
