Here in Wokingham we are a lovely team with a real sense of team spirit and we are looking for someone who shares our values to join our 0-19 service.

Whatever your experience we will ensure you receive individualised and effective support from both your health visiting line manager and colleagues within your team. If you have not done your prescribing course is is something we can support with.

You will work within the Bracknell area to deliver core and targeted services to children aged 0-5 and their families.

For this role you will require excellent communication and interpersonal skills as well as a passion for working with children and families.

Team working, flexibility and the ability to work in partnership with other teams and agencies is essential. You will collaborate with Children’s Centre’s in the implementation of the Healthy Child programme underpinned by the Solihull Approach model of working.

If successful, your base will be Wokingham Community Hospital ; however, your role will require you to visit homes across Berkshire (mileage will be compensated).

### Main duties of the job

Some of the general responsibilities include.

- Develop and implement outcome focused care plans and evaluate care given, in partnership with clients in accordance with the Healthy Child Programme.

- Provide support to families using the Solihull approach model in behaviour management, parenting skills and delivering programmes of care to vulnerable families.

- New birth visits, targeted and safeguarding work in in collaboration with our partners, supporting parents with infant feeding, attendance at strategy and multi-professional meetings.
- Develop and facilitate Health Visiting drop-in clinics, health promotion activities and groups.

- You will be a Non-medical prescriber or willing to undertake this training which will be fully supported.

- You will hold a valid UK driving license with access to a vehicle.
