Are you a School Nurse, Specialist Community Public Health Nurse (SCPHN)?

An exciting opportunity has arisen to join our School Nursing Team in Hemel Hempstead and St Albans.

This role will enable you to fulfill your Public Health role delivering the 5-19 Healthy Child Programme (HCP) as well as working with SCPHNs, Team Leaders and Clinical Leads across Hertfordshire to continually improve the quality of service offered to children and young people. Hertfordshire Community Trust (HCT) are committed to developing the skills of all nurses and offer a range of leadership courses as well as restorative and safeguarding supervision.

This post is also suitable for newly qualified SCPHN School Nurse students.

HCT will prioritise recruitment of workers living in England under the new immigration rules for care workers effective from 9 April 2025. ### Main duties of the job

To lead and supervise members of the Public Health Nursing 5-19 team to deliver universal, targeted and specialist care to children and young people, following the Healthy Child and Young People Public Health Nursing Programme.

The post holder will work with children, young people and families within the school and community setting or within the home environment, as deemed necessary, to provide and promote the healthcare needs of children and young people in Hertfordshire to achieve to their full potential and thrive into adulthood.

The post holder will have an integral part in Safeguarding children and young people, providing health assessments for Children Looked After, and Safeguarded children, following appropriate safeguarding processes. The post holder will also cover 'duty', assess, triage, plan, and will implement healthcare as appropriate in accordance with Trust Guidelines and the Healthy Child Programme.

Demonstrate an evidence-based approach to practice to continue the delivery and development of a high standard of service, whilst supporting the Team Lead and Clinical Quality Lead. Responsibilities will also include but not limited to, attending local liaison meetings, undertaking health promotion/education activities, completing appraisals and one to one’s.

Promote effective team working, sharing knowledge and information, encouraging a supportive environment, role modelling equality, diversity, and inclusivity to promote good working practice and outcomes.

### Detailed job description and main responsibilities

Our service:

- Is committed to delivering a high quality service to our children, young people and families as a 0-19 service.
- Work with individual children and young people as well as developing local public health plans with schools and the wider community
- We are an innovative service that embraces the use of technology
- Delivers the Healthy Child Programme by using an electronic health needs assessment process to identify individuals requiring early intervention pathways
- Uses Chat health text messaging service for 11-19 year old's and a text messaging service for parent / carer's
- Has implemented two health websites for use by children, young people, families & professionals.
- Has developed and promoted the use of social media to connect with children, young people and their families.

The successful candidate will:

- Be a confident, independent practitioner
- Be a skilled team player and able to lead a skill mix team
- Have excellent communication skills to use with a variety of clients, professionals & staff
- Have experience of electronic record keeping (this would be an advantage although full training will be given)
- Have a full valid driving license and access to a car to use for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
- Require an enhanced child and adult DBS certificate

### Qualifications

- Registered Nurse/ direct entry Midwifery qualification.
- Specialist Community Public Health Nurse (School Nursing)
- Current Registration with NMC
- Mentorship and preceptorship/ENB 998 or other relevant recognised teaching/assessment qualification or willingness to undertake.
- Nurse prescribing qualification(V100,V150)
