The service covers the London Borough of Wandsworth and provides complex care for people with Heart failure involving supporting secondary and primary care, community integrated services and other healthcare professionals. Our specialist nurses are required to deliver complex Heart Failure management together with providing education to patients to improve self-management of their condition. ### Main duties of the job The Heart Failure Specialist Nurse will primarily be responsible for managing a caseload of patients with Heart Failure in the community. To  support this, the Heart Failure Specialist Nurse will liaise with other members of the heart failure management team across primary and secondary care to deliver evidence based care to patients with Heart Failure. The Heart Failure Specialist nurse will also co-ordinate an education programme for community health professionals and act as a resource for advice.
