We have an exciting opportunity for you to join our established Community Heart Failure as a specialist nurse for a years Fixed term or secondment.

The service covers the London Borough of Wandsworth and provides complex care for people with Heart failure involving supporting secondary and primary care, community integrated services and other healthcare professionals.

Our specialist nurses are required to deliver complex Heart Failure management together with providing education to patients to improve self-management of their condition.

### Main duties of the job

The Heart Failure Specialist Nurse will primarily be responsible for managing a caseload of patients with Heart Failure in the community.

To  support this, the Heart Failure Specialist Nurse will liaise with other members of the heart failure management team across primary and secondary care to deliver evidence based care to patients with Heart Failure.

The Heart Failure Specialist nurse will also co-ordinate an education programme for community health professionals and act as a resource for advice.

To use specialist Heart Failure knowledge and skills to safely identify, maintain and manage a caseload of heart failure patients, providing them with appropriate treatment, support and advice.

- Monitor Heart Failure patients immediately following discharge in order to prevent readmission.
- To contribute to the development of care pathways for people with Heart Failure, ensuring appropriate NICE guidelines are implemented.
- To undertake comprehensive assessments of complex Heart Failure patients with co- morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning.
- To accurately undertake specialist clinical skills including administration of medication within National and local Guidance

## Person Specification

### Skills

**Essential**

- Ability to undertake leadership role in clinical specialty
- Advanced skills in the assessment and treatment of clients with Cardiology issues
- Ability to work as a team member and promote collaborative multi-disciplinary and multi-agency working.
- - Advanced knowledge of evidence-based practice associated with Cardiac conditions.
- Ability to interpret health and social policies and guidelines and to translate into practice.
- Ability to problem solve, prioritise and demonstrate clinical reasoning based on experience and knowledge.

### Experience

**Essential**

- Experience working in Heart Failure or cardiology with a special interest in Heart Failure either in primary or secondary care & managing patients
- Experienced in managing a clinical case load
- Experience in the training of students
- Experience in organising and implementing training/teaching programmes for healthcare professionals and clients
- Excellent group education and presentation skills & and ability to present to professionals at all levels.
- Experience of conducting audits and/or research
- Experience in contributing to Service Development, including policy and guideline development
- Experience in supervising others
- Experience working in a multi-disciplinary team

**Desirable**

- Experience working in Community settings
- Experience with working with change behaviour techniques

### Qualifications

**Essential**

- Registered Nurse with full and current UK NMC registration
- Post basic qualification in cardio-respiratory care or equivalent experience
- Qualification in Independent nurse prescribing or willing to undertake training.
- Hold, working towards or willing to undertake relevant qualification in physical assessment

**Desirable**

- Has relevant degree or evidence of working towards degree
