Location: Pharmacy Team, Aberdeen City HSCP.

Aberdeen City Health and Social Care Partnership (HSCP) has an exciting opportunity for an experienced pharmacist to join our team and play a key role in the ongoing delivery and development of the pharmacotherapy service across Aberdeen City.

This is a full-time vacancy, however, part time may be considered with a minimum of 21.6 hours per week.

Working alongside pharmacist and technician colleagues, and with support from HSCP Lead Pharmacists and Pharmacy Team Co-ordinators, the successful candidate will work across GP practice(s) and the pharmacy hub. The role will cover all aspects of medicines management and pharmacotherapy, ensuring the safe, appropriate, and cost-effective use of medicines to deliver high-quality pharmaceutical care for patients.

The post holder will also contribute to the supervision, training and mentoring of wider pharmacy team members, supporting workforce development within the service.

As an Advanced Pharmacist, you will lead, oversee and further develop key elements of pharmacotherapy within allocated GP practices, including:

  • Medication and polypharmacy reviews
  • Medicines reconciliation
  • Actioning hospital discharge letters
  • Management of acute and repeat prescription requests
  • Providing prescribing advice to the multidisciplinary team
  • Responding to ad-hoc clinical queries
  • Participation in multidisciplinary team (MDT) meetings
  • Supporting social care colleagues with medication-related issues, including care at home services

This post will particularly suit pharmacists with extensive experience in any sector who are looking to progress into an advanced primary care role, combining clinical expertise with leadership, supervision and service development responsibilities.

Essential Criteria:

  • MPharm / Degree in Pharmacy
  • Registration with the General Pharmaceutical Council (GPhC)
  • Independent Prescriber (IP)
  • Completion of a clinical diploma / Advanced Competency Framework (or equivalent knowledge gained through experience)

Applicants should clearly demonstrate how they meet the essential criteria within their application, particularly where knowledge has been gained through experience rather than formal qualifications.

  • Strong IT skills
  • Excellent communication and interpersonal skills
  • Ability to work flexibly across Aberdeen City
