It supports hospital discharges, prevents admission to hospital or long-term care and promotes independent living. Its a multi-disciplinary service with Nurses, Physiotherapists, Occupational Therapist, Social Workers, and Care Staff. Please note that Care Plus Group are not currently registered to sponsor applicants from outside the EU and EEA. ### Main duties of the job Title: Social Worker Internal applicants only Base: Community Inpatient Unit Hours: 37.5 a week, permanent, pay award pending Salary Range 5 £29,317 - £35,715 per annum As a social worker within the team at CIU you would work: As part of a wider multi-disciplinary team to coordinate and support admissions and discharges within the unit. To assess care needs and to implement appropriate care package/ placement within the Care Act Legislation. To act as a role model in the adherence of the Mental Capacity Act.
