### Job summary

NHS Cheshire and Merseyside pathfinder scheme will centralise the commissioning and case management arrangements for patients with complex extended rehabilitation needs across Merseyside and Warrington.

The service will provide specialist support for patients, carers and colleagues across the region, ensuring patients are identified, transferred and case managed through the most appropriate pathway to meet their rehabilitation needs.

### Main duties of the job

The role of the social worker within the case manager team is to provide a trusted assessor role on behalf of locality partners within the designated caseload. The social worker will complete Care Act Assessments, Continuing Health Care triggers and provide expert social care and signposting for patients and families as required.

The social worker role is key to efficient discharges within the IS pathway of care, ensuring safe and effective plans are in place whilst considering appropriate legal and funding frameworks.

### Qualifications

**Essential**

- Degree or equivalent in Diploma in Social Work, or equivalent recognised by the Central Council for Education and Training in Social Work.

### Skills and Attributes

**Essential**

- Demonstrate effective problem solving skills and ability to respond to sudden unexpected demands.
- Excellent negotiation skills with the ability to offer constructive challenge.
- Ability to produce concise and accurate verbal and written reports in accordance with professional body guidelines and ICB policy and protocol
- Excellent team working skills coupled with the ability to work autonomously and prioritise own workload.
- Demonstrate sound IT skills.

**Desirable**

- Social Work England registration.
- Understanding and commitment to the principles, practices and promotion of equality and diversity.

### Knowledge and Experience

**Essential**

- Extensive post-registration experience relevant to the clinical area.
- Experience of working within a multi-disciplinary team (MDT) and chairing MDT meetings or panels.
- Provision and receipt of highly complex, sensitive or contentious information, negotiating with stakeholders as required.
- Presenting complex and sensitive information to a range of recipients.
- Demonstrate the ability to perform to a high level in a clinical setting, holding services to account and ensuring effective use of NHS resources.
- Carrying out risk assessments and managing risk.
- Knowledge and understanding of the philosophies of Safeguarding of Vulnerable Adults and children
- Detailed knowledge of the Care Act, Mental Capacity Act (MCA) and Mental Health Act (MHA) and a clear understanding of the interface between these.
- Detailed knowledge and understanding of the background to and aims of current health and care policy areas and the implications of this on commissioners, providers and key stakeholders.
- Knowledge of legal issues pertaining to complex care client group.
- Budget monitoring and working to Standard Financial Instructions.

- Experience of working with patients with complex rehabilitation needs on an extended pathway.
- Experience of supervising student social workers
