The service will provide specialist support for patients, carers and colleagues across the region, ensuring patients are identified, transferred and case managed through the most appropriate pathway to meet their rehabilitation needs. ### Main duties of the job The role of the social worker within the case manager team is to provide a trusted assessor role on behalf of locality partners within the designated caseload. The social worker will complete Care Act Assessments, Continuing Health Care triggers and provide expert social care and signposting for patients and families as required.
