This rewarding role focuses on supporting children, young people and families, particularly those with Learning Disabilities (LD) and/or Special Educational Needs and Disabilities (SEND), to improve their health and wellbeing through personalised, non-clinical support. Working within a Primary Care Network (PCN), you will help families identify what matters most to them, navigate local services, access community support, and build confidence and resilience. You will work collaboratively with GP practices, schools, Family Hubs, Children's Services and voluntary sector organisations to ensure families receive timely, coordinated and inclusive support. The successful candidate will be passionate about improving outcomes for children and families, have excellent communication skills and be able to build strong relationships across a range of services and communities. You will be working within a team supporting a population of 85,000 patients in Gosport that are registered with Bridgemary Medical Centre, Blossom Health, Rowner Medical Centre, Solent View Medical Practice and The Willow Group GP Surgeries. ### Main duties of the job Receive and manage referrals from GP practices, schools, Family Hubs, Children's Services and other partner organisations. Provide personalised, strengths-based support to children, young people, families and carers, with a particular focus on SEND and Learning Disability needs. Support families to understand and navigate local health, education, social care and community services. Develop personalised support plans based on individual goals, strengths and priorities. Connect families with appropriate community groups, activities and services to improve health, wellbeing and social inclusion. Work collaboratively with schools, SENCOs, Family Hubs, GP practices and voluntary sector organisations to coordinate support and reduce duplication. Facilitate access to practical and emotional support that helps build confidence, resilience and independence. Promote social prescribing within the local community and develop strong relationships with local services and community groups. Maintain an up-to-date knowledge of local services, resources and support available to children, young people and families. Manage a caseload effectively, maintaining accurate records and monitoring outcomes. Contribute to safeguarding processes and work within local safeguarding policies and procedures.
