While the base post is on Ward 12 as Ward Sister, this role sits within the wider frailty pathway, meaning there may be opportunities to work across different areas within the pathway. As a key member of the team, you will play a vital role in delivering high-quality care to patients requiring support for frailty-related conditions. This is an excellent opportunity for an experienced, motivated, and enthusiastic nurse with a strong interest in caring for patients with frailty needs. The Frailty Pathway includes AFU, Ward 12, and Sunderland Ward. There will be opportunities to rotate across these areas for defined periods, supporting both professional development and service delivery. Ward 12 is a 24-bedded ward with a friendly and supportive team, committed to delivering high-quality nursing care ### Main duties of the job Promote a ward culture that prioritises the prevention of deconditioning by embedding meaningful, person-centred activities into daily care. Uphold the Health Board’s values of respect, fairness, integrity, honesty, openness, kindness, compassion, and care in all interactions, and actively encourage these behaviours within the team. Deliver compassionate, high-quality, and effective patient-centred nursing care, alongside fair, transparent, and accountable people management practices. Provide visible and professional nursing leadership in line with NMC standards, offering expert clinical advice, support, supervision, and guidance. Ensure all care delivery and management processes comply with legal, professional, and organisational requirements, and are grounded in evidence-based practice. Foster a positive learning environment that supports the continuous professional development of nursing staff, students, and the wider multidisciplinary team, enabling them to deliver safe, effective care that maintains patient independence and wellbeing.
