### JOB SUMMARY

The post holder will be responsible for delivering to the strategic priorities relating to the delivery of safe and effective discharge targets from acute and non-acute hospitals across the Southern HSC Trust. They will provide leadership and have managerial responsibility for the delivery of a high-quality service to achieve safe and effective discharge planning and the promotion of improved health and well-being outcomes for patients.

The post holder will be responsible for working with key stakeholders across the acute and community hospital settings and will take the lead in improving pathways and systems in relation to patient flow. This will involve working across all Directorates within the Southern Trust and across other HSC Trust boundaries to ensure timely, safe and effective discharges for patients/clients and putting in place systems to measure outcomes. The post holder will work closely with the Senior Managers and will provide operational and professional supervision to staff.

### KEY DUTIES / RESPONSIBILITIES

To provide leadership to a multi-disciplinary team and manage the delivery of high quality safe and effective discharge planning in conjunction with key internal and external stakeholders in order to promote improved health and well-being outcomes for patients and clients.

Operational Management

Be accountable for the daily operational management of the discharge team.

Support senior leadership with the development, implementation and evaluation of all discharge processes

To ensure the effective management of discharge processes

To develop a process to identify and manage demand and capacity daily.

To manage the operational process for complex discharge across a 7-day service.

Utilise a project management approach to lead on and implement service development, improvement, and modernisation to engage all staff and deliver sustainable changes in culture and working practice.

Have a level of whole-system knowledge that facilitates communication at all levels including departmental managers, senior nursing staff, senior clinicians and managers across the Trust, external care providers, external agencies and local community providers.

Through the process of observation, facilitated workshops, learn from complaint and compliments to establish diverse ways of working aiming to improve patient flow across the Trust

Understanding and implementing legislation and policy to ensure effective use of health resources at all times

Leads by example in the development of shared vision and values for the services and promotes a culture where all the staff understand, and engage in and working towards, the achievement of identified KPIs

Input and update data related to Complex Medically optimised patients to provide daily report and updates.

Work with colleagues across multiple organisations to resolve problems.

To ensure effective working relationships are established and maintained with colleagues across the Trust.

Staff Management

Management of multi-disciplinary team including line management, HR processes including absence and performance management and supporting the continuous development of the individuals and team.

Develop and maintain robust and meaningful relationships with clinical and non-clinical divisions at a senior level.

Provide education and support to staff and other departments pertaining to complex patient discharge.

Performance Management

To ensure that services provided are of a consistently high quality and deliver excellent patient experience.

Lead the investigation and timely response to formal complaints as required, collaborating with clinical staff to ensure that the lessons are learnt from issues and systems and processes are amended to ensure that these issues are not repeated.

Collate and analyse complex discharge information, to provide accurate assessments of service capacity and demand across the health and social care economy.

Assist Head of Service to provide information and action plans for complex discharge.

To assist the Head of Service in capacity and demand planning, negotiating and the successful delivery of performance metrics

Risk and Clinical Governance

Support the Head of Service with implementing effective systems, control processes and risk management arrangements in line with the clinical and corporate governance strategy

Responsible for the development of guidelines and Standard Operating Policies related to discharge processes, along with the implementation of all related policies by the Discharge Team to ensure clear appropriation of Trust policies.

Lead on incident reporting and education of staff following incidents, and adverse events. To ensure that incidents recorded on Datix are appropriately actioned.

Regularly assess risks to health, safety and security using the results to promote and improve practice.

Be aware of the safeguarding agenda. Role model best practice in line with the policy, ensuring protection and identification of vulnerable adults.

To liaise with senior staff and take appropriate action to address staffing issues when the need arises.

Ensure that policies and procedures are applied, and systems are in place to promote patient and staff safety

Service Development/Improvement

Lead on creating improvement, transformation and innovation in the role of discharging patients

Possesses project management skills, utilising action plans and has the ability to work to deadlines and tight timescales

Identifies barriers to effective communication and develops/implement strategies to overcome these, enabling users/patients to participate as partners in their care planning

Uses specialist knowledge to actively lead in managing change by acting upon audit and other reviews to implement agreed new approaches to raise the quality of care in the relevant clinical areas and in collaboration with other health care professionals

Supports and facilitates the education and training needs of staff
