### JOB SUMMARY

The Home Treatment/Crisis Response Team Leaders will provide Leadership, supervision, support and day-to-day management of the Home Treatment/Crisis Response Team.

The postholder will have clinical role within the service acting as a role model to other members of staff. The role will encompass facilitating the implementation of the service model, research, practice development, audit and supervision of junior staff.

The Team Leader will be responsible for ensuring appropriate induction and training for those employed in the team. The postholder will also be responsible for contributing to the development of performance management and service evaluation systems.

This post includes responsibility for partnership working with the Trusts Mental Health Services colleagues as well as providing support to service users and carers. This will involve working collaboratively on a broad range of operational and service development issues.

The postholder will promote effective multidisciplinary working and work in partnership with other agencies to deliver a comprehensive range of Home Treatment/Crisis Response services. The postholder will engage in the care management process for the mental health service user if required.

### KEY DUTIES / RESPONSIBILITIES

Manage the multidisciplinary team, monitoring the workload of the team ensuring that professional standards are maintained in all areas of practice.

Develop and manage staff rotas to include planning for annual leave and other absences and ensuring an adequate skill mix of staff are available to deliver the service effectively.

To work in partnership with other team leaders within Home Treatment Crisis Response and allocate resources as required to ensure the service is delivered area wide.

Allocate work to team members and supervise the delivery of the range of specific functions within the team;

Supervise and maintain the safe custody, control and administration of drugs and take appropriate action/measures which promote safe administration of medicines.

Ensure that all resources deployed by the team are specific and based on assessed need, and that they are delivered, monitored and reviewed in line with policy and procedure.

Have an overview of all open cases within the team.

Record, collate and analyse all information in order to identify a total picture of service user/carer needs.

Ensure written plans and risk plans and risk assessments are formulated and delivered and reviewed.

Recognise and respect the rights of the service user in their care provided.

Ensure the necessary information is available to enable service users to make informed choices in all aspects of their care.

Ensure the service meets the needs of the service users’ cultural/religious beliefs, recognising the personal preferences of the patient.

Ensure that users and carers are involved in all aspects of the service maintaining privacy/dignity/safety as well as ensuring that they maintain/develop their personal/social support contacts.

Ensure the involvement of relatives/friends/advocates in the planning and implementation of patients’ care, if this is the wish of the service user.

Be a member of the Trusts Mental Health Management team and attend other meetings and committees as required, deputising for Home Treatment Co-ordinator his/her discretion.

Promote collaborative working between locality teams and ensure that consistent advice and consultation is available across teams.

Initiate, lead or attend as appropriate any clinical meeting that may be required and ensure that there are effective systems of communication, support and feedback in the team and all relevant stakeholders.

Chair the multidisciplinary team clinical and business meetings.

Keep up to date with all current and national legislation that affects mental health services and continually update skills in line with professional guidelines.

Ensure that own and other practice, within the line management framework, is evident based.

Provision of the required induction training for all new members of the Home Treatment/Crisis Response Team.

Be responsible for the support, development, supervision and performance review of staff.

Work collaboratively with Trust colleagues to identify training needs both unidisciplinary and multidisciplinary within the Home Treatment/Crisis Response Team.

Promote the Home Treatment/Crisis Response service by facilitating a learning environment and liaise with staff from training institutions where appropriate.

Provide perceptors and mentors for students on placement with the team and ensure that they receive experiences and training in order for them to meet their training needs.

Co-ordinate placement of students within the team, ensure the service is an approved/accredited placement area with appropriate support practice supervisors.

Participate and lead as appropriate in post qualification training and education.

Investigate complaints and serious incidents in keeping with the relevant procedures and produce reports to the required standard.

Ensure that self and all staff adhere to relevant codes of conduct and advisory documents published by governing bodies e.g. NMC/NIPEC and NISCC, Trust Policies/Procedures, legislation, DHSSPS Policies and Procedures.

Ensure that all professional staff maintain their professional qualifications and registration status.

Participate in the development of Service Level Agreements such as to ensure a quality service.

Contribute and fully participate in the Trust Corporate and Clinical & Social Care Governance arrangements, including Audit, Re-Audit, Complaints Management Continuous Service Improvement, Risk Management and Life long learning.

Attend and contribute to meetings within each professional discipline to discuss contemporary issues and share good practice.

Ensure that the service is reviewed, monitored and continuously improved in line with the framework of clinical governance.

Participate in the planning process within the Trust so that the Trust Delivery Plan and corporate objectives are responsive to user and care needs.

Evaluate all proposed packages of care and ensure that all services specified are available within existing resource constraints and that authorisation is obtained prior to the commitment of resources.

Monitor and control the use of any other financial resources deployed by the team.

Take responsibility for effective budget and resource management including Human Resources.

Support the Administration and Clerical manager in developing systems to support data and performance management requirements.

Produce clear, precise and timely reports using the Trust IT systems.

Be responsible for managing and resolving difficult or high-risk situations along with or in the absence of the Consultant, taking the appropriate management action.

Take an active role in ensuring working conditions are as pleasant as possible and any unpleasant conditions are reported appropriately.

Take appropriate steps to reduce harm to self and others.
