### JOB SUMMARY

The advanced practitioner physiotherapy for frailty services will have responsibility for organising and ensuring delivery of a physiotherapist service to patients who are identified as frail, within all general acute wards and ED within the acute setting.

The post holder will work at an advanced clinical specialist level, maintaining responsibility for the provision of clinical leadership and expert advice regarding referrals to the acute physiotherapy service for elderly / frail patients, and to provide feedback to the team lead.

Clinical leadership and expert guidance should be provided to other physiotherapists, consultants/registrars, patient flow and all other members of the multi-disciplinary team within the acute sector and across community interfaces with specific guidance and training for the management of the more complex frailty cases. The post holder will work to ensure best intervention and management of patients and to agree validated outcome measures for this service. Where concerns are raised communication will occur directly with referrers.

The post holder will work with the acute patient flow team / acute social work team to maximise the flow of patients across the hospital site and into the community, focusing on the prevention of admission where appropriate and the facilitation of discharge. This will include assessing more complex patients to ensure holistic management and safe and timely discharge from the acute site, making best use of non-acute / community resources.

To actively lead in the development and monitoring of protocols in line with changing practice. To implement policy and service changes to working practices and evaluating the service in conjunction with National, professional and multidisciplinary standards of care.

The post holder will work with the Lead Physiotherapist in Acute In-Patient Physiotherapy service, the Professional Head of Physiotherapy and the Operational Head of AHPs to ensure the strategic development of the physiotherapy service in SHSCT and across the region, developing and implementing policies and procedures.

The post holder will be required to assist in the daily management, supervision and co-ordination of staff and to deputise for the team lead as required.

### KEY DUTIES / RESPONSIBILITIES

### OPERATIONAL DELIVERY

To gain informed consent and undertake physiotherapeutic assessment of patients referred for physiotherapy, to diagnose and provide appropriate treatment to facilitate patient discharge.

To provide specialised treatment interventions as an expert clinician, for elderly / frail patients referred for physiotherapy in ED and general wards, within the acute setting.

To provide advice to physiotherapy staff, consultants, registrars and other members of the multi-disciplinary team re management of complex patients and to agree validated outcome measures.

To identify and establish solutions in proactive discharge planning alongside the multi-disciplinary team, and provide support with complex discharges.

To participate in the monitoring and evaluation of discharge performance

To motivate patients to maximize their rehabilitation potential, promote a degree of self-management, and make informed lifestyle changes to improve health related quality of life.

To deliver a specialised physiotherapy service in the acute directorate and be responsible for the assessment and treatment of this client group (frailty). To ensure safe and effective discharge planning and appropriate onward referral.

To plan, deliver and evaluate patient centred interventions to meet the needs of ‘users’ and ensure effective outcomes.

To complete accurate comprehensive records in line with Trust and professional standards on assessment findings, diagnosis, investigations and physiotherapy intervention.

To monitor and provide expert advice on Geriatric Liaison Service within the acute setting and inform the physiotherapy team lead and Consultants of changing needs within the service, dealing directly with service issues as they arise.

To actively participate in the Emergency On Call Rota / 7 day working rota as required

### COMMUNICATIONS

To communicate relevant clinical information impacting on diagnosis and prognosis of patients to Consultants, GP’s and other members of the multi-disciplinary team promoting multi-disciplinary working to ensure the holistic needs of the patients / carers are met.

To communicate information effectively with patients and carers re their condition- where expectations are unrealistic to help them come to terms with the management of their symptoms or life limiting condition.

Act a resource to colleagues being recognised as a clinical expert providing advice on complex cases.

To further develop personal communication and presentation skills, to facilitate the regular in-house training of other disciplines providing a service to this client group.

To represent the physiotherapy service at multi-disciplinary meetings, working groups and special interest groups locally and regionally.

### KEY WORKING RELATIONSHIPS

To be responsible for the day-to-day management and supervision of physiotherapy staff and physiotherapy support staff. .

To be responsible for the setting and reviewing rotational objectives for physiotherapy staff and assistants and encourage clinical development through regular supervision, mentoring, and reflective practice (including preceptorship and probationary periods as appropriate).

To provide a high standard of patient care and ensure all other staff does likewise and monitors individual caseloads to ensure that these correlate with the grade of staff.

To be responsible for facilitating physiotherapy student clinical placements on a regular basis – promoting learning through reflective practice whilst giving them advice, support and teaching on the clinical management of patients.

Ensure adequate arrangements are in place for the induction of any new staff to the Physiotherapy service and contribute to the induction of other professionals and volunteers

To Adhere to The Chartered Society of Physiotherapy Standards, Code of Ethics and Professional Conduct and ensure own competency to practice through CPD activities. Maintain a C.P.D. folder to ensure continued State Registration with the Health & Care Professions Council.

### HEALTH AND SAFETY

Have a working knowledge and adhere to all the principles of the Health & Safety at work policy.

Assist the team lead in monitoring and evaluating equipment needs and changing patterns within the Acute service. Report all defects of equipment to the team lead and takes appropriate action.

### SERVICE IMPROVEMENT

To develop protocols and treatment pathways, and implement policy and service developments which will result in changes to team working practices to result in improved outcomes and access to services.

To lead on audits, evaluation of projects and research, to enhance service provision and to set and quality assure clinical uni-professional and multi-professional standards as required.

Be aware of the changing needs of users and commissioners, and together with the team lead develop the service to meet local, area and regional targets.

To provide accurate statistical information to allow the service to be maintained, monitored and audited

### QUALITY

To assist in the development and delivery of clinical and social care governance.

### TRAINING & DEVELOPMENT

To lead on and provide specialist advice, training and teaching appropriate to all bands of staff, to ensure staff competency and development.

To plan, co-ordinate and deliver training for weekend working and set up and review buddying systems to ensure staff competency in carrying out weekend duties.

To plan and deliver post-graduate training to physiotherapy staff and the multi-disciplinary team on a regular basis training and attend all other relevant in-service training.

To participate in alternative learning methods to ensure staff are up to date with best practice e.g. e learning, CSP clinical interest groups.

Offer clinical advice and support across the physiotherapy service to acute staff and staff in other directorates, linking evidence-based practice to the clinical management of conditions.

To complete designated post-graduate, clinical and mandatory training to ensure further development of diagnostic and clinical skills.

To be up-to-date with post graduate clinical research and evidence based practice and ensure dissemination of findings to other clinicians and managers. To become involved in any regional trials / research into the management of older persons conditions.
