### JOB SUMMARY

Within the policies and directives of SHSCT the post holder will help develop, provide, maintain and co-ordinate an effective range of appropriate services for service users with a Sensory Disability to achieve or maintain a mainstream tenancy.

The primary purpose is to support the person to discharge their responsibilities as a Tenant or home owner and by doing so maximise personal independence, provide integration into the community and improve the quality of life of those he/she comes into contact with.

### KEY DUTIES / RESPONSIBILITIES

To receive referrals and carry out an assessment of the needs of service users who have a Sensory Disability in relation to housing.

Develop in partnership with the service user support plans with clearly identified goals and objectives.

To operate within the policies of the Southern Health and Social Care Trust, Supporting People and NISCC, the objective being to maximise personal independence, integration into the community and improve quality of life of those service users he/she comes into contact with.

Contribute to the Quality Assessment Framework document for Supporting People.

To help tenants maintain the security of their dwelling.

To assess for risks within the home and immediate home environment and where appropriate enable tenants to maintain a safe living environment.

To liaise with other professionals and agencies in the assessment of and management of risk in the home environment.

To help tenants comply with their responsibility as householder e.g. helping with housing and social security benefits, pay rent and household bills etc.

To help tenants maintain the dwelling in an appropriate condition. This will be particularly relevant to people with sight difficulties and the worker will liaise closely with the Rehabilitation Worker for people with sight difficulties.

To work as a member of the Sensory Disability Team, attend and contribute to monthly Team meetings.

Maintain records in accordance with Trust policy.

Participate in training programmes within the Trust, including Sign Language training, which may involve attendance at evening classes. To undertake minimum level 2 BSL and develop a range of communication skills.

Take responsibility for managing workload and establishing priorities within it.

Participate in the Trust’s staff appraisal and personal development as defined by Trust policy.

Participate in monthly supervision with the Team Manager in accordance with the Trust current supervision policy.

Undertake office duty on a rotational basis with the Team.

The following are essential criterial which will initially be measured at Shortlisting Stage although may also be further explored during the interview stage:

Hold NVQ/QCF level 3 Diploma in Health & Social Care Support or equivalent.

At least 6 months paid experience working with adults with Sensory Loss, (Profoundly Deaf, Hard of Hearing, Deaf Blind, Visually Impaired, Sight Impaired or Severally Sight Impaired).

Obtained Level 1 BSL Sign Language or attending Level 1 BSL training.

DESIRABLE CRITERIA - these will only be used where it is necessary to introduce additional job related criteria to ensure files are manageable. Evidence of at least 6 months experience of working with adults with Sensory Disability (Profoundly Deaf, Hard of Hearing, Deaf Blind, Visually Impaired, Sight Impaired or Severally Sight Impaired).

As part of the Recruitment & Selection process, it may be necessary for the Trust to carry out an Enhanced Disclosure Check through Access NI before any appointment to this post, can be confirmed.
