The Daisy Hill Day Clinical Centre (DCC) provides planned outpatient treatments such as intravenous (IV) biologic treatments; blood, magnesium and iron transfusions; and venesections for conditions like arthritis and inflammatory bowel disease. DCC also facilitates planned day case procedures such as DC cardioversions, biopsies and fibro-scans.

The post holder will also be required to manage patients who require a consultant-led review/ follow-up investigations post discharge, which in turn, helps reduce hospital readmission rates and improves patient safety, experience and satisfaction.

### JOB SUMMARY

The post holder provides advanced, autonomous nursing care and leadership within the unit. The role combines clinical expertise, service development, patient education, and multidisciplinary collaboration across a number of medical specialty services.

The post holder will ensure effective and safe day to day operational management of the unit. The post holder is expected to carry out all relevant forms of care and procedures for which they are competent to practice without a direct supervision. The post holder will also ensure the development of a collaborative approach across multiple specialties to support the development of referral and treatment pathways that will complement the enhancement of ambulatory and same day emergency care pathways on the DHH site.

The post holder will also work closely with the Assistant Service Manager to further develop service provision within the unit with a clear focus on patient safety, service quality and timely provision of care.

### KEY DUTIES / RESPONSIBILITIES

### CLINICAL PRACTICE

Act as an autonomous nursing clinical practitioner within a nurse provided service, undertaking advanced holistic assessment at key points in the patient pathway as required. This could include physical, emotional, psychological and social needs and the development of appropriate plans of care;

Provide continuous ongoing assessment of information and support needs of patients, carers and families, signposting to other sources of information and support, referring to other specialty services when required;

Develop, deliver and evaluate new models of review, which are risk weighted and needs tailored to maximise benefit to patients and the service;

Triage all referrals to the unit ensuring that only planned outpatient treatment and day case procedures with the agreed scope of practice are delivered in the unit;

Act as resources to all registered nurses working in the unit on matter related to the forms of care, treatment and procedures delivered within the unit ensuring that nursing staff have available to them current, relevant and evidence based information on which to base their nursing decisions;

Maintain awareness of patients starting systematic therapies or biologics, including monitoring, counselling and safety assessments;

Liaise and collaborate with the multidisciplinary team across medical specialties to maintain the continued delivery of holistic care to patients;

Play a key role in the multidisciplinary team, consistent with the roles outlined in the Improving Outcomes Guidelines, including contributing to service improvement within the team;

Establish systems and processes to ensure effective communication and continuity of patient care;

To engage regionally and nationally in the development and implementation of multi-professional standards and clinical management guidelines for the care of patients attending the unit;

Advocate for patients and ensure that their views inform service development;

Implement and monitor compliance with clinical standards to measure the outcome of prescribed care;

Maintain, track and improve Nursing Quality Indicators applicable to the unit;

Ensure a safe environment for patient care, underpinned by best practice guidelines and Health and Safety policies, including the use of risk assessment and the development of action plans to mitigate/ eliminate identified risks;

Undertake quality assurance audits on an agreed basis to establish patient satisfaction;

Have a flexible approach to own working hours in direct relation to service delivery needs.

### EVIDENCE-BASED PRACTICE

Promote a culture of research and evidence-based practice within the unit to enhance person-centred care;

Maintain and develop competency in facilitating a wide range of critical enquiry techniques to support the development of reflection in and on practice;

Maintain awareness of current developments related to the various specialties that interface with the unit and the implications these developments may have on their nursing practice;

Create and maintain a working environment conducive to research and practice development, encouraging staff within the unit to participate in specific aspects of research related to nursing care using approved methods;

Maintain and develop competency in facilitating a wide range of critical enquiry

Ensure evidence-based care is provided to agreed standards within the unit;

Act as a change agent, developing clinically effective practice through the effective utilisation and integration of recognised best practice;

Participate in developing, implementing and monitoring policies, procedures and protocols;

Create an environment which supports a culture of life-long learning and reflective practice for all staff;

Ensure systems and processes are in place to support effective mentoring and peer-to-peer learning.

### COMMUNICATION

Play an integral role in the development and improvement of services within the unit to ensure that they are efficient, effective and comply with best practice service standards;

Review processes /practices including those within the ward to support patients/clients to improve their own health and well-being;

Review processes/practices to ascertain if there are better ways of working within the ward/department to enhance patient/client care, service delivery and deliver required efficiencies;

### LEADERSHIP, QUALITY AND GOVERNANCE

Act as a visible leader within the unit;

Provide leadership that enables professional decision making and effective team working;

Empower and enable staff to contribute to the delivery of high quality person-centred care;

Support clinical expertise of nurses within the unit by role modelling evidence-based person-centred care;

Lead and participate in annual staff appraisal/development review, ensuring effective implementation of the Knowledge and Skills Framework (DH, 2004);

Lead, support and develop nursing health care support workers within the unit;

Promote a person-centred culture to facilitate good staff relationships and morale among staff;

Contribute to the education commissioning process for nursing staff;

Work in partnership with the Nursing Workforce and Education Team to ensure that nursing staff have access to commissioned courses to support the professional and personal development;

Ensure supervision and mentorship is embedded for pre and post registration students, registered nurses and health care support workers, as appropriate;

Challenge appropriately and confidently where standards of care fall below that which the Nurse Manager would expect;

Locally resolve any concerns or complaints of patients or others where possible by early intervention, and to investigate formal written complaints in line with the HSC Model Handling of Complaints Procedure, forwarding a response to the Assistant Service Manager (ASM) in the timeframe required;

Work with the ASM to develop action plan using lessons learned from complaints and incidents;

Manage poor performance and practice of staff in line with SHSCT Trust policies;

Ensure appropriate delegation to nurses and monitoring of outcomes for effective care/service delivery;

Manage and report accidents, incidents or hazards according to SHSCT policies and procedures;

### FINANCIAL RESOURCE MANAGEMENT

Contribute to budget management within the ward/department, to ensure services are managed in accordance with yearly financial instructions;

Participate in multi-professional procurement processes to ensure appropriate selection of products which meet relevant quality and safety standards;

Deliver a safe and effective service within allocated resources, ensuring the resources are used to maximum effect;

Monitor budgetary performance, develop and implement action plans, organise the necessary resources and monitor outcomes;

Adhere to financial policies and procedures, particularly Standing Financial Instructions, Authorisation Frameworks, Procurement Legislation and associated processes, and Prompt Payment Code;

Adhere to HSC Trust financial controls and fraud awareness principles (e.g. verification of authenticity / accuracy of the Staff-in-Post records);

### STAFF MANAGEMENT

Ensure nurses working in the unit are professionally qualified and registered with the NMC;

Ensure nurses working in the unit achieve compliance with statutory and mandatory training requirements;

Liaise with the Lead Nurse and the Divisional Nurse on all professional nursing issues;

Ensure processes are in place to manage sickness/absenteeism and take appropriate action in line with HSC Trust policies;

Promote the health and well-being of staff and observe for any signs of ill health or stress factors in staff assigned to the area and take appropriate action in line with HSC Trust policies and NMC guidelines;

Adhere to good rostering practices in managing duty rotas, annual leave and study leave to ensure adequate nursing cover and appropriate skill mix at each shift;

Participate in the recruitment and selection of staff, up to and including Band 6.
