This includes working alongside therapists and other colleagues to provide coordinated, high-quality rehabilitation. You will also contribute to supporting discharge from hospital by delivering timely rehabilitation interventions in patients' own homes where appropriate. You will support the management of a varied caseload under the direction of a qualified therapist, including patients with complex needs, and contribute to service development and quality improvement initiatives. You will be supported by experienced community therapists and will follow guidance and, where appropriate, complete assessments of individuals in their own homes to maximise their functional outcomes.
