We are looking for enthusiastic and person-centred senior registered nurses to join our team. The multi-disciplinary IST is highly skilled, passionate and completely person-centred, comprising of Nursing, Occupational Therapy, Psychology and Speech & Language Therapy. The team’s overall aim is to provide dynamic support to people living in the community and prevent an admission to hospital / placement breakdown. The team work across Durham, Tees Valley to support people with learning disabilities who have experienced physical and emotional trauma and may have additional mental health needs, sensory processing difficulties and/or communication needs. The team has 3 workstreams Home Support, Wrap around and Provider. This advert is for the wrap around support stream (please note that this is a community post but there is an expectation that all staff will be trained to PAT Level 2 - restraint trained to support the wraparound pathway - please consider before applying). Staff working in our wrap around stream will have the opportunity to work with people with complex needs supporting individuals and the MDT to bring complex packages of care to life, meet people’s needs in the right way, improve their quality of life and prevent admission to hospital. Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework. ### Main duties of the job This role requires 7 day working between the hours of 8am and 8pm. Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010. The successful applicant needs significant experience in working with adults with learning disabilities, mental health difficulties and/or complex needs. The bases are within both Durham and Teesside and can be negotiated on appointment to the team. As a senior registered nurse in the team, you will support a small caseload of people and the MDT around them. You will also support a dedicated wrap around clinical nurse specialist in the development of wrap around plans.
