### Job overview

We are looking for an enthusiastic band 6 Physiotherapist to work within the Community Rehabilitation Team at Home First.

The Community Rehabilitation team supports people following admission to hospital to regain their independence at home. They are supported by the Rehabilitation Assistants to complete their personal hygiene and meal tasks. A large majority of these patients have orthopaedic conditions and need assessment and treatment to improve their mobility and range of movement.

You should be personable with excellent communication skills. You will be expected to work alone and within the multi-disciplinary team. You should be able to organise and prioritise your own workload and balance this with the demands of this exciting role.

We will provide support through in-house professional forums, Mandatory training, 1:1 and peer supervision. We facilitate student education and have close working relationships with neighbouring teams in both acute Health and Social Care.

You should be HCPC registered, have appropriate demonstrable post qualification experience of working within in rehabilitation, be a car driver and have access to a car.

### Main duties of the job

- To provide specialist assessment, diagnosis and treatment of patients with complex Physiotherapy problems as an autonomous practitioner within clinical speciality area.
- To undertake line management responsibility for staff, within the service area to provide a responsive and effective service.
- To actively support the Team Leader in the operational management of the area and service developments on a local and/or service-wide level.
- To provide clinical education and training.
- To have responsibility for specific projects or initiatives contributing to the overall delivery of the service within a clinical speciality area.
- To be responsible for providing information to the Senior Management Team

### Skills

**Essential**

- Ability to communicate complex information clearly to a variety of professionals and to patients and their families, overcoming barriers to communication.
- Demonstrable skills in assessment, clinical reasoning and problem solving for complex case management
- Able to maintain judgement under pressure
- Ability to delegate effectively
- Positive approach to problem solving
- Proven organisational skills including time management and prioritisation to manage clinical and administrative workload
- Confident user of computers and IT databases. Proficient in Word, Excel and Power point.
- Demonstrate flexible approach to meet needs of patients and service delivery
- Able to recognise own limitations, when to seek supervision, and implementation of reflective practice

**Desirable**

- Understanding of the Home First pathways.
- Completed student practice placement educator course
- Experience of project work at departmental level
- Ability to work in collaboration and positively influence or coach others.

### Knowledge

**Essential**

- Understanding of current NHS issues and initiatives.
- Awareness of current relevant clinical guidelines e.g. Falls guidelines.
- Demonstrate clinical knowledge of a broad range of diagnoses and conditions in line with areas of the post
- Understanding of the issues pertaining to clinical risk management, and ability for formulate plans to reduce risk
- Demonstrable knowledge HCPC standard of practice and of professional registration requirements

### Experience

**Essential**

- Demonstrable appropriate post graduate experience as a Band 5 physiotherapist in a broad range of areas including adequate relevant experience in the specific clinical specialty.
- Evidence of relevant CPD
- Evidence of community working
- Experience of working with older adults.
- Experience of supervising junior / support staff.
- An understanding of the principles of clinical governance
- Able to demonstrate an understanding of professional conduct and confidentiality
- Extensive experience of working as part of a physiotherapy and multidisciplinary team (MDT)
- Clinical education and development of physiotherapy staff and other members of the MDT.

**Desirable**

- Experience of NHS working
- Experience in undertaking personal development appraisal.
- Leadership experience.
- Experience of research, audit, reflective practice and outcome measures.
- Experience of working with patients with a history of falls, and long term conditions and palliative conditions.
- Experience of networking within physiotherapy and in the wider Health & Social Care community

- Diploma/Degree in Physiotherapy
