Are you an experienced and motivated Senior Physiotherapist looking for an exciting opportunity to make a real difference in patients' lives while developing your leadership and advanced clinical skills?

We are seeking a passionate Band 7  Senior Physiotherapist to join our innovative and supportive Harrow Urgent Community Response Team on a 9 month fixed term maternity cover contract.

Our Urgent Community Response Service delivers high quality, person centred care directly in people's homes, helping to prevent unnecessary hospital admissions and supporting adults aged 18 and over to remain safe and independent in their own communities. This is a rewarding opportunity to work at the forefront of community healthcare, providing rapid assessment, intervention, rehabilitation, and clinical decision making within a multidisciplinary team.

The service operates 7 days a week, 365 days a year between 8:00am and 10:00pm. This role includes participation in weekend and bank holiday working on a rotational basis, with a commitment to promoting a healthy work life balance wherever possible.

### Main duties of the job

- To undertake comprehensive Head to Toe assessments without supervision and to determine patient need/risk and plan, implement and evaluate their care with support from service consultant.
- In the absence of senior colleagues, take charge of the unit, day to day management of the team including taking handovers and helping to make treatment plans at the virtual ward round with the consultant.
- To work in partnership with multi-disciplinary members of the team, other professionals, and agencies, to meet the health and rehabilitation needs of patients within the Brent Urgent Community Response Team .
- To maintain standards of clinical care using an evidence-based approach within the service, incorporating principles and values of clinical governance
- Ensure effective communication is always maintained, whilst providing support, supervision, and clinical expertise to all members of the team
- Develop your skills into a more well-rounded holistic approach to patient care.
- Take on extended roles such as venepuncture, drug administration, advance assessment of the acutely ill patient and acute injury assessment.

Must hold UK HCPC registration and  Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

**Essential**

- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

### Experience

**Essential**

- Experience working at a senior level in a wide range of clinical areas including, neurology, general rehabilitation, falls, frailty, elderly medicine.
- Extensive experience in Primary/Secondary Care and/ or Accident & Emergency
- Proven experience in utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and manage patients presenting with undifferentiated and undiagnosed conditions.
- Experience in assessing and providing quality care to people with Long Term Conditions
- Track record of practice excellence.
- Experience in effectively working in collaboration, negotiating, and liaising across organisational boundaries.
- Experience in monitoring clinical standards.
- Proven ability to prioritise and manage own workload with minimal supervision.
- Experience determining the best ongoing care for the patient.

**Desirable**

- Experience of wide range of specialist services eg. falls services, wheelchair services, and learning disabilities.
- Experience in team/service/ staff management.
- Experience working as a Band 6 Physiotherapist
- Experience of community rehabilitation
- Experience of working in the NHS

### Skills & Knowledge

**Essential**

- Skills in the assessment and treatment of adults with complex therapy needs
- Knowledge and understanding of complex medical conditions including ie. neurology, multi-pathologies and falls
- Knowledge of role of Allied Health Professionals and interdisciplinary team working
- Ability to apply core physiotherapy skills across a range of clinical conditions within a client centered, goal setting
- Knowledge of outcome measures and evidence based practice relevant to Physiotherapy practice in community
- Knowledge of the NICE falls guidelines, National Stroke Strategy, Clinical Guidelines for Stroke and the National Service Framework for Long-term Conditions
- Ability to problem solve, prioritise and demonstrate clinical reasoning
- Teaching of junior staff / students, peers in in service training setting
- Knowledge and understanding of the role of Social Services and voluntary agencies

**Desirable**

- Supervision of assistant / junior staff
- Knowledge and experience in research and audit
- Knowledge and understanding of National Service Framework for Older People, NICE Guidance and Clinical Guidelines for progressive conditions

### Education/Qualification

**Essential**

- Diploma/degree in Physiotherapy
- Current HCPC registration
- Evidence of up to date CPD portfolio
- Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation or older people

**Desirable**

- Post graduate degree in relevant area
- Further desirable qualifications in Primary Care/ Accident & Emergency/ acute clinical assessment.
